Care Continuum Manager (LVN)

HealthView Hospice & Palliative Care | HealthView Home Health Services Field-based — Los Angeles County| Full-Time | [Pay - Salary - 80K a year ]

Job Summary

HealthView is an integrated hospice, palliative care, and home health organization serving Los Angeles, Orange, San Bernardino, and Riverside Counties. We are seeking a Care Continuum Manager (CCM) a licensed LVN embedded in our partner assisted living communities (ALFs/RCFEs) as the familiar face residents, families, and facility staff see every week. On-site weekly at each assigned community, the CCM runs our proactive care model: Assess → Identify → Engage → Intervene → Prevent — catching clinical decline weeks early, not mid-crisis, and working before the 911 call.

This is not a desk job and not reactive case management. You are the single point of contact between your communities and HealthView's full clinical organization, backed by a 24/7 on-call clinical team.

Job Duties

Conduct clinical rounds alongside facility nursing staff using validated tools (FAST scoring, PPS, pain and symptom screening) to identify residents showing decline or ,approaching hospice or home health eligibility, before a hospitalization
Complete resident binder audits on every visit: consents, physician orders, care plans, advance directives, and election documentation, so partner communities stay survey-ready at all times
Track compliance requirements per resident and per service; identify gaps in writing before a surveyor does
Ensure hospice and home health care plans align with facility care plans
Lead family engagement conversations for high-eligibility residents while residents are still stable and able to participate in their own care decisions
Serve as physician liaison: communicate clinical changes and secure orders and certifications
Coordinate hospital-to-facility transitions with no documentation gaps
Deliver a monthly eligibility and compliance report to each community's DON and administrator
Provide in-service education to facility staff on hospice eligibility criteria, documentation standards, and communication protocols
Coordinate referrals with HealthView's clinical team, with hospice or home health at the bedside within 24–48 hours

Experience

Active, unrestricted California LVN license (required)
Current BLS/CPR certification (required)
Experience in hospice, home health, assisted living/RCFE, skilled nursing, or geriatric care strongly preferred
Familiarity with Title 22 RCFE regulations and survey processes a significant plus
Working knowledge of FAST and PPS scales, or willingness to be trained
Strong documentation skills and EHR proficiency; solid command of HIPAA requirements
Exceptional relationship skills: You'll be the trusted, consistent presence for residents, families, facility nurses, administrators, and physicians
Comfort with autonomy and time management across multiple assigned communities
Valid California driver's license, current auto insurance, and reliable transportation

Pay: $80,000.00 per year
