Full Job Description

Job Overview
We are seeking a dedicated and compassionate Staff Nurse with their LPN to join our dynamic healthcare team at Hellenic Senior Living. In this vital role, you will provide exceptional patient care across various clinical settings, ensuring comfort, safety, and well-being for all individuals under your supervision. Your expertise will support the delivery of high-quality nursing services, promote health education, and foster a positive environment for patients and their families. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations while advancing your nursing career.

Responsibilities

· Participate in the development and implementation of resident plans of service, and service plan conferences, performing within scope of practice.

· Discuss care plans with the nurse manager or supervisor on a regular basis.

· Receive reports and assignments from the Resident Service Coordinator, and give daily reports to on-coming staff or the Supervisor.

· Oversee assignments for aides for timely completion.

· Assure that care delivered is appropriately documented in the clinical record.

· Monitor resident appearance for good personal hygiene.

· Assure that a resident’s environment is neat, orderly, and safe.

· Implement care and treatments according to accepted Home Health and /or Residential clinical practices, policies and procedures.

· Carry out physicians’ orders and evaluate resident outcomes appropriately.

· Complete assignment sheets for the aides that reflect resident assessments and service plans.

· Assure that clinical record documentation is according to appropriate clinical standards.

· Assure that services are documented in a manner that enables appropriate billing, when applicable.

· Promote the protection and respect for the privacy of resident information.

· Recognize and address changes in a resident’s condition as evidenced by physical symptoms, verbal expression, or changes in mental or psychosocial status.

· Communicate recommendations for additional resident services to the appropriate individuals in a timely manner.

· Participate in staff meetings and committee meetings to foster communication of the inter-disciplinary team, policies and procedures, and changes in operations, and to promote a positive work environment.

· Assure that resident or family concerns and complaints are handled according to complaint policy and procedures.

· Consult with the Resident Service Coordinator or supervisor as issues and concerns require.

· Be trained on and promote efficient use of equipment and supplies.

· Report malfunctioning or unsafe equipment and inadequate supplies to the appropriate individual.

· Help with the initial orientation and on-going educational training of other nurses and aides.

· Participate in educational programs for personal growth and professional development and to maintain current knowledge of nursing care standards and operations.

· Assume assigned responsibilities as the nurse manager or supervisor in their absence, including but not limited to, assisting the Executive Director with administrative functions, supervising care and services, and being on-call for emergencies.

· May be asked to be available or on-call for emergencies or other situations that develop at the facility.

· Provide oversight to QMAs/CNAs in their assigned areas.

Experience

· Has current license as a Registered or Licensed Practical Nurse in Indiana and is a graduate of an accredited school of nursing. Evidence of independent nursing practice in delivering nursing care is required.

· Experience in long-term care or public health nursing, or equivalent experience in a hospital, retirement community, or nursing facility preferred.

· Ability to read, write, and make simple mathematical calculations.

· Ability to understand and carry out instructions that are provided in written, oral, or diagram form.

· Well-developed communication and interpersonal skills.

· Demonstrate expertise in a variety of long-term care clinical concepts, practices and procedures.

· Possess experience and common-sense judgment to discern solutions to problems and professionally respond to issues.

· Ability to get along well with diverse personalities; tactful, mature and flexible.

· Ability to establish credibility and empower supporting members of management and staff.

· Must meet the state’s licensing requirements for the class of the vehicle being operated and have a valid license.

· Ability to maintain good attendance and attends mandatory in-service meetings.

· Ability to work overtime or on weekends if needed.

Physical Requirements/Working Conditions:

Ability to remain stationary 50-100%; position self to exert 10-30 lbs. of force or to move objects; move about to access office cabinets, machinery, etc.; operate a computer or other office machinery; frequently communicate and exchange accurate information; perceive visual acuity to operate office machinery; work around varying internal and outdoor environmental conditions such as temperature changes, noise, movement, atmospheric conditions, etc.. Well-lighted environment with a smoke-free policy. Work at the Nurses’ Station and throughout the building. Subject to a variety of climate environments in residents’ rooms/apartments. May be subject to infections--trained in hand-washing practices, use of protective equipment, and appropriate precautions to minimize exposure. May lift and transfer residents-- training in body mechanics and lifting techniques designed to prevent personal injury. Subject to interruptions and noise on a daily basis. Work at a computer terminal but trained in safe work practices to prevent injury.

Pay: $31.00 - $32.50 per hour
