Company Overview

TEAM NY coordinates the delivery of home care services to Chinese-American patients in New York City and Long Island through a network of three Licensed Home Care Service Agencies: ABI Healthcare, ATD Home Health Agency, and Life Improvement Homecare.

We are one of the largest and longest-standing home care organizations serving the Chinese-American community in New York. Our mission is simple but powerful: help seniors age with dignity, independence, and support in the comfort of their homes.

As our patients’ needs continue to grow, we are building something new: a Patient Concierge service designed to provide trusted, high-touch support beyond traditional home care coordination.

Role Summary

Many seniors and families do not leave an agency because of care alone — they leave because they feel unsupported, confused, or on their own, this Patient Concierge will help change that.

We are looking for someone who is warm, resourceful, and highly service-oriented to become the first hire in our new Concierge organization. In this role, you will help making our patients feel that TEAM NY is not just their home care agency, but their trusted partner. By helping with the small things that matter every day, you will directly improve patient loyalty, family satisfaction, and the quality of life of the communities we serve.

This is more than a customer service role. As the first hire in this function, you will play a key role in building a new patient experience program from the ground up, and you will have the opportunity to grow with the program and potentially become a future manager as the Concierge team expands.

What You’ll Do

Provide High-Touch Patient Support

Serve as a trusted point of contact for patients and families who need help with non-clinical, everyday support needs. Listen carefully, ask thoughtful questions, and make patients feel comfortable, respected, and cared for.

Help Patients Navigate Daily Life

Assist patients with tasks such as reading and explaining English-language letters, translating basic information into Chinese, helping understand appointment reminders, organizing paperwork, and guiding patients through simple next steps.

Schedule and Coordinate Appointments

Help patients schedule doctor visits, transportation, follow-ups, community appointments, and other important services. Confirm details clearly and help patients feel prepared.

Provide Basic Tech Assistance

Support patients and families with simple technology needs, such as using phones, video calls, patient portals, text messages, online forms, apps, or other tools they may find confusing.

Connect Patients with Community Resources

Research and connect patients with helpful community services, senior programs, food resources, transportation options, benefits support, social activities, and other resources that may improve their quality of life.

Support Patients Virtually and In Person

Handle many requests by phone, but provide in-person support when needed, including office-based support or occasional community/patient-facing assistance.

Build Trust and Loyalty

Create a premium, caring experience that makes patients and families feel supported beyond the basics. Help strengthen patient retention by becoming someone families trust and remember.

Help Build the Concierge Program

As the first hire in this function, help create the playbook. Track common requests, identify recurring patient needs, suggest process improvements, and help shape how the Concierge team grows over time.

Partner Across Teams

Work closely with care coordinators, intake, operations, compliance, caregivers, and leadership to make sure patient needs are handled appropriately and escalated when needed.

What You’ll Bring

High school diploma or GED required; Associate’s or Bachelor’s degree preferred.
Fluency in English and Mandarin and/or Cantonese required
Prior experience in customer service, healthcare, home care, social services, hospitality, concierge services, community support, or patient-facing work preferred.
Strong patience, empathy, and respect for seniors
Clear communication and strong phone skills
Strong organizational and follow-up skills.
Comfort using computers, phones, messaging tools, and online systems
Resourceful mindset — you know how to ask questions, research, and solve problems
Strong judgment to know when to escalate an issue to the right internal team
Ability to work in-office 5 days per week in Flushing, NY.
Willingness to provide occasional in-person patient, office, or community support as needed.
