Full Job Description

Forensic Supportive Housing Specialist

Location: Glen Cove (2 days/week) & Baldwin (3 days/week), NY
Travel: Regular community-based travel throughout Nassau County
Schedule: Full-Time | Monday-Friday, 9:00 AM-5:00 PM
Flexible schedule required to meet program needs, including participation in an on-call rotation.

Compensation

Non-Bachelor's Degree: $24.72/hour
Bachelor's Degree or Higher: $27.00/hour

At Charles Evans Center (CEC Healthcare), we believe stable housing is the foundation for recovery, independence, and long-term success. We are seeking a compassionate and dedicated Forensic Supportive Housing Specialist to join our Supportive Housing Program.

In this role, you will work alongside individuals with behavioral health challenges and justice-involved backgrounds as they build meaningful lives in the community. Through advocacy, skill-building, care coordination, and supportive services, you will help residents maintain housing stability, achieve personal goals, and strengthen their connection to the communities where they live.

What You'll Do

As a Forensic Supportive Housing Specialist, you will:

Provide supportive housing services in accordance with OMH regulations and program standards
Help residents develop independent living and life-management skills
Support individuals with housing stability, apartment maintenance, budgeting, and community engagement
Conduct assessments and identify areas where additional support and skill development are needed
Coordinate services with treatment providers, community agencies, landlords, and other key partners
Advocate for residents and assist them in navigating community resources
Maintain accurate, timely, and compliant documentation of services and resident progress
Provide crisis intervention, problem-solving, and supportive guidance when needed
Participate in an on-call rotation to support residents outside of regular business hours
Travel throughout Nassau County to meet residents in their homes and communities

Required Qualifications

Associate degree or equivalent college credits
Experience working with forensic populations and/or individuals with complex behavioral health needs
Strong written and verbal communication skills
Valid New York State driver's license in good standing with a clean driving record
Daily access to a reliable vehicle for work-related travel
Ability and willingness to participate in an on-call rotation

Preferred Qualifications

Bachelor's degree in Social Work, Human Services, Psychology, Criminal Justice, or a related field
Experience in supportive housing, case management, care coordination, or community-based services
Familiarity with Electronic Health Record (EHR) systems
Proficiency with Microsoft Office applications
