Chronic Care Coordinator

Work Location: Brooklyn, Boro Park, NY

Work Setting: Onsite (4Days in Office and Fridays Remote)

NSIPA provides a comprehensive suite of services, including credentialing and network operations, provider engagement, and contracting; care coordination and member outreach; and provider-and patient centered programs. These services are designed to strengthen provider efficiency, address health related social care needs, and advance value‑based care and population health opportunities. Position Overview:

We are seeking a highly organized and detail‑oriented Chronic Care Coordinator to support our care coordination team. This non‑clinical position focuses on patient outreach, documentation management, and chart review. The ideal candidate brings experience in chronic care documentation, strong communication skills, and proficiency with electronic health record (EHR) systems. This is a Hybrid position.

Roles & Responsibilities:

· Accurately upload and manage patient documentation within EHR systems

· Conduct thorough chart reviews to ensure accuracy, completeness, and compliance.

· Identify and report missing or outdated documentation to the appropriate care team members.

· Communicate professionally with patients to support continuity of care, follow-ups, and coordination needs.

· Collaborate with clinical staff to maintain current and accurate patient records.

· Ensure full compliance with HIPAA and internal confidentiality standards.

· Bilingual in English and Russian is preferred.

Minimum Requirement and Qualifications:

· Minimum of 1 years of experience as a medical assistant.

· Prior experience with chart review, documentation, and chronic care management is mandatory.

· Proficiency with EHR/EMR systems.

· Strong attention to detail, organization, and time management skills.

· Ability to work independently and manage remote work flows effectively.

· Knowledge of CMS chronic care management billing codes (preferred)

· Personal computer and reliable high speed internet connection is required, and equipment is not provided for this role.
