Full Job Description

Position Summary:

The ECM Clinical Lead Care Manager will lead the screening, assessment, treatment planning and ongoing care management for participants experiencing acute or chronic mental health or substance abuse disorders. The Care Manager utilizes evidence-based screening and assessment tools and therapeutic techniques to stabilize, support and engage participants to better manage their own chronic conditions. The Clinical Lead Care Manager actively engages other team participants including Care Managers, SUD Navigators, and Community Health Workers to remove barriers to health, social services, and behavioral health care.

Qualifications:

Experience:

· Four years of direct care management experience including working independently in the field, conducting biopsychosocial assessment, developing care plans, and utilizing evidence-based treatment modalities to improve functioning.

· Experience completing evidence-based mental health and substance abuse screenings and assessments.

· Experience working with underserved and diverse populations in community settings.

· Demonstrated ability to support, coach and motivate participants to reach their goals.

· Valid California driver’s license, reliable transportation and proof of current liability insurance required.

· Ability to work effectively from home daily (private workspace, stable internet connection, etc.).

· Ability to manage time effectively in order to complete tasks and meet deadlines.

· Experience using electronic technology (laptop, tablet, cell phone) and Microsoft Office Suite (Excel, Word, and Outlook) required.

Education:

· Master’s degree from an Accredited School of Social Work.

· Licensed Clinical Social Worker certification from the State Board of Behavioral Sciences

required.

Responsibilities:

Participant Eligibility and Assessment:

· Confirm ember’s eligibility by verifying qualifying chronic condition and acuity criteria with participant.

· Complete comprehensive mental health and substance abuse disorder clinical screenings, assessment and treatment planning utilizing evidence-based practices.

· Determine diagnoses if qualified.

Participant Focused:

· Assess participant’s motivation to change using Readiness to Change scale and use interventions appropriate to participant’s Readiness to Change.

· Utilize evidence-based practice therapeutic techniques to engage participant and support participant’s motivation to reduce symptoms and better manage their overall health.

· Act as lead consultant for participants in mental health crisis, suicide risk screening and intervention and acute substance abuse.

· Recommend treatment approaches to care team participants to effectively manage participants’ mental health or substance abuse disorder symptoms.

· Coordinate with the care team and monitor ongoing referrals to community resources and services to support participant’s access to health, behavioral health and SDoH resources.

· Identify, assess, and complete Child Protective Services and Adult Protective Services referrals per state mandated reporter requirements.

· Communicate with participant’s health plan and service providers to ensure coordination of services and to advocate for participant’s needs and preferences.

Participant Education:

· Provide support and education to participants to assist them in adhering to medical recommendations and taking medications as prescribed.

· Provide system utilization education.

· Assist participants in accessing and understanding necessary health and wellness information provided by their medical provider and clinic.

Quality Management:

· Complete data needed for daily/monthly reporting by date assigned by supervisor.

· Complete corrections from audit to remain in compliance with program standards provided in a timely manner.

· Conduct regular case audits including reviewing assessment, care plan and service notes and provide feedback to Care Manager and CHW as appropriate to ensure overall quality of case documentation.

Ongoing tasks:

· Guide Interdisciplinary Care Team (ICT) meetings to ensure the quality and appropriateness of services being provided to the participant and to better support participant’s ability access to needed health care services and community resources.

· Documentation of activities in care coordination database.

· Complete all telephone and face to face contacts with participants in a timely and professional manner.

· Complete participant-centered treatment goals in collaboration with participants and review progress in meeting goals routinely.

· Provide regular and timely updates to medical providers, referral sources, and assigned clinics on participants' progress to meet treatment goals and to trouble shoot any problems or concerns.

· Complete all required documentation and data tracking in a detailed and timely manner.

· Attend and actively participate in weekly supervision, trainings and meetings as scheduled to ensure staff’s understanding and ability to apply best practices and evidence-based practices while working with participants.

Physical Demands:

· While performing the duties of this job, the employee is regularly required to identify, communicate, and exchange information.

· Expected to travel to community meetings, participant homes, or other agencies.

· May experience traumatic situations including psychiatric, traumatized, and deceased participants.

· Employee may regularly need to lift up to ten (10) pounds.
