Full Job Description

Position: Mobile Mental Health Therapist

Type: Part-Time

Location: LECOM Institute for Behavioral Health (LIBH)

The Mobile Mental Health Therapist provides a variety of therapeutic services to older adults living with mental health challenges and a physical or mental health diagnosis that limits them from being able to receive services in the community.

The position is responsible for conducting mobile mental health intakes and assessments, diagnosing mental health disorders, developing personalized treatment plans for individuals to address and manage their mental health concerns, and delivering mental health therapy. This position requires the ability to provide intensive case coordination and therapeutic services primarily, but not limited to, individuals with an emotional disturbance or mental illness at the senior care facilities across LECOM Health.

Responsibilities:

As a Mobile Mental Health Therapist, your role will include but is not limited to: intake and assessment services which include administering intake interviews and intake procedures, conducting comprehensive evaluations, completing biopsychosocial assessment, developing strengths-based and individualized treatment plans; therapeutic services which include providing evidenced-based therapeutic interventions services according to the individualized treatment plan, continually monitor progress and re-evaluate patient needs in collaboration with the treatment team, assisting patients receiving care and their treatment teams with implementing, monitoring and re-evaluating crisis and safety plans, responding to emergencies with appropriate de-escalation techniques and intervention plans, facilitating group therapy sessions (if applicable), providing flexible scheduling to best meet the needs of the patient; and case management services which include coordinating treatment with other service providers, assistance to patient’s family in locating needed resources and client advocacy, helping patients to link to community resources, working collaboratively with the treatment team and serve as the liaison between patients and medical staff, providing support and guidance during transitions of care and building relationships with community partners to enhance service delivery.

Requirements:

· A graduate degree from a college or university that is accredited by an agency recognized by the United States Department of Education or the Council for Higher Education Accreditation (CHEA) in a generally recognized clinical discipline in which the degree program includes a clinical practicum.

· An equivalent degree from a foreign college or university that has been evaluated by the Association of International Credential Evaluators, Inc. (AICE) or the National Association of Credential Evaluation Services (NACES). The Department will accept a general equivalency report from the listed evaluator agencies to verify a foreign degree or its equivalency.

· Drivers License and insurance

Preferred:

· Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW).

· A minimum of 1 year of experience working with adults and/or older adults with mental health diagnosis.
