Full Job Description

Job description:
We are seeking a dynamic and dedicated Outreach Specialist to join our team. The Homeless Outreach Specialist is dedicated to reaching out to individuals experiencing homelessness in various environments, including encampments, streets, places unsuitable for habitation, meal programs, and day centers. The Homeless Outreach Specialist role specializes in supporting homeless individuals with serious mental illnesses or co-occurring substance use disorders. The primary objective is to connect clients with vital resources and services to help stabilize their housing, mental health, and overall well-being.

Responsibilities:

Conduct outreach to individuals experiencing homelessness in diverse settings such as streets, encampments, places not intended for habitation, day centers, and meal programs.
Provide crisis intervention through interviews, assessments, and counseling, addressing social, emotional, financial, health, or other issues.
Implement evidence-based practices like trauma-informed care, motivational interviewing, and harm reduction during assessments and treatment.
Collaborate with team members and community partners to overcome obstacles to permanent housing, including securing identification, legal assistance, benefits, income support, and developing life skills.
Create personalized care plans that focus on the client's strengths and are tailored to their specific needs.
Operate independently, effectively managing time between community outreach with clients and fulfilling program documentation requirements.
Other duties as assigned.

Qualifications:

Bachelor's diploma or higher.
2 years relevant experience involving similar vulnerable populations.
Excellent communication and public speaking skills.
Valid PA driver’s license required.
Knowledge of addiction counseling and public health is preferred.

Pay: From $23.69 per hour
