Job Overview

We are seeking a dedicated and compassionate Direct Support Professional and Home Health Aide (HHA) to join our team. The ideal candidate will possess a strong commitment to patient-centered care, with experience working with diverse populations including seniors, individuals with developmental disabilities, and those requiring memory or hospice care. This role offers an opportunity to make a meaningful difference by supporting clients with their daily activities and medical needs in a respectful and dignified manner.

Duties

Provide personal care assistance including bathing, dressing, grooming, and mobility support using patient transfer methods such as Hoyer lifts when necessary.
Assist clients with Activities of Daily Living (ADLs) such as meal preparation, light housekeeping, laundry, and medication reminders while maintaining a clean and safe environment.
Monitor patient health by observing vital signs, reporting changes, and documenting medical information accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
Support clients with specialized needs including Alzheimer's disease, dementia, memory care, and end-of-life care, ensuring comfort and dignity at all times.
Administer medications following home healthcare regulations and ensure compliance with HIPAA standards to maintain patient confidentiality.
Communicate effectively with clients and their families to provide updates on health status and address concerns promptly.
Follow protocols for emergency response including first aid, CPR, basic life support (BLS), and hospice care procedures when necessary.

Qualifications

Prior experience as a Home Health Aide or in assisted living, nursing home, or long-term care environments is highly preferred.
Knowledge of medical terminology, catheter care, medication administration, and vital signs monitoring.
Experience working with individuals with developmental disabilities, geriatrics, dementia, or memory care needs.
Familiarity with home health care assistance regulations and documentation practices using EMR/EHR systems.
Certification in first aid and CPR is required; additional training in hospice or end-of-life care is advantageous.
Ability to lift heavy objects safely and perform physical tasks associated with patient transfer methods such as Hoyer lifts.
Strong communication skills coupled with compassion for providing patient personal care assistance in a respectful manner.
