Full Job Description

PRIMARY PURPOSE -

This position serves as a critical connector between unhoused individuals and the continuum of care available in Lane County. The primary responsibility of this role is to conduct outreach across all St. Vincent de Paul homeless service locations and guide clients to the Schlies Resource Center, where they can access coordinated services through on-site community partners and providers. This position will work closely with community partners, service providers, and internal teams to ensure individuals receive comprehensive and timely support.

2. ESSENTIAL DUTIES AND RESPONSIBILITIES -

· Outreach and Engagement

· Conduct regular, in-person outreach at St. Vincent de Paul homeless service locations, including shelters, safe parking sites, and drop-in centers.

· Establish and maintain positive, trauma-informed relationships with individuals experiencing homelessness or housing instability.

· Educate clients about available services at the Schlies Resource Center and encourage their participation in scheduled appointments with external service providers.

· Connection to Schlies Resource Center Services

· Coordinate with clients to schedule appointments with community partners offering services at the Schlies Resource Center (e.g., Department of Human Services, Senior and Disability Services, Goodwill Job Connections).

· Assessment and Navigation

· Conduct needs assessments with clients to determine their eligibility and readiness for specific services.

· Assist clients with navigating service systems offered at the Schlies Resource Center, including initial appointments and follow-ups.

· Service Coordination

· Collaborate closely with internal St. Vincent de Paul teams and external partner agencies to ensure a coordinated and efficient response to client needs.

· Maintain regular contact with key community partners (e.g., outside agencies providing services through SRC and case managers).

· Advocate for client access to appropriate services and ensure warm hand-offs when connecting to providers.

· Documentation and Data Management

· Accurately document client interactions, outreach contacts, and service plans using internal tools and assessments, Lane County’s Homeless Management Information System (HMIS), and other agency systems.

· Maintain confidential and up-to-date, accurate case notes, appointment tracking, and service outcomes in compliance with HIPAA and organizational policies.

· Track and report outreach outcomes, trends, and gaps in service to support program development.

· Program Support and Team Participation

· Work closely with and support Client Connection Specialist.

· Participate in outreach planning, case conferencing, staff meetings, and trainings.

· Contribute to the improvement of service delivery by providing feedback from the field.

· Represent St. Vincent de Paul’s mission and values in all community interactions.

· Build relationships and rapport with community partners.

· Communicate effectively and respectfully within the context of varying beliefs, behaviors, orientations, identities, cultural backgrounds regarding clients, staff and community partners.

· Maintain professionalism, safe, respectful and productive environment for clients, staff and volunteers.

· Utilize de-escalation and trauma-informed approaches with a variety of vulnerable clients and uphold the mission and values of SVdP.

3. OTHER DUTIES AND RESPONSIBILITIES -

· Support additional homeless services or outreach efforts as needed to ensure individuals receive holistic and accessible care.

· Maintain adherence to Company policies, safety/ergonomic standards and good housekeeping practices.

· Provide coverage as needed for other teams within the SRC and the HSS department.

· Perform other duties and responsibilities as required.

4. MINIMUM QUALIFICATIONS AND EXPERIENCE -

· Previous work experience in social services, case management, outreach or related field.

· Strong knowledge of community resources and services for unhoused individuals.

· Excellent communication and interpersonal skills with the ability to build trust and rapport.

· Ability to work in a fast paced, client centered environment with individual facing trauma, mental health challenges, and substance use issues.

· Proficiency in data entry, record keeping and service tracking.

· Understanding of harm reduction, trauma-informed care and equity-based service delivery.

· Possess a valid driver’s license and in good standing with SVdP’s driving policy.

· Preferred Qualifications:
