An older adult gets discharged and you already know they'll be back in 30 days. A family leaves overwhelmed, clutching a stack of papers with numbers they'll never get through. You watched the handoff crumble—the PCP never got the discharge summary, the home health agency showed up unprepared, the family had no idea what questions to ask.

You did everything you could within the system and it still wasn't enough. If you've felt frustrated by the lack of coordination between providers—and want to actually fix it—Hera is for you. We started Hera because we felt the system was letting down the generation that raised us.

What is Hera?

Hera is dedicated care management in the community for older adults, covered by Medicare. We’re depth over breadth and intend to be with families until end-of-life.

Every family gets matched with a dedicated Hero, you, who actually has the tools, time, and autonomy to help. It’s not a 15 min monthly check-in. It’s real, ongoing support where you see things through and families call you their Hero.

The role

Must reside in California
1099, 30–40 hrs/week in business hours EST (you set your schedule)
$30–60 / hour: factors include experience, geography, and client load
Serve elderly adults with chronic conditions and their families: educate on navigating Medicare and the healthcare system, coordinate care across providers, advocate for what they need

You’d be a great fit if you:

Hold an RN (clinical license a plus but not required)
Background in case management, home health, hospice, geriatrics, or long-term care
Well-rounded experience across multiple care settings is a plus
Love fostering connection with older adults and their families
Are a creative problem solver who wants a job that looks different every day
Are tech-savvy and excited by quickly evolving environments
Remote - must be located in the United States
