Overview

Join our dynamic healthcare team as a Home Health Registered Nurse and make a meaningful difference in patients' lives by providing compassionate, expert care in the comfort of their homes. This vital role offers an exciting opportunity to utilize your extensive clinical skills, including critical care, trauma, ICU, and post-acute care experience, to deliver personalized patient assessments, coordinate care plans, and promote recovery and well-being. As a key member of our home health team, you will collaborate with healthcare professionals to ensure seamless transitions from hospital to home and support patients through every stage of their recovery journey.

Responsibilities

Conduct comprehensive patient assessments, including vital signs, physiological evaluations, and medical history reviews to determine individualized care needs.
Develop and implement tailored care plans that align with medical orders, patient goals, and healthcare regulations.
Manage patient care in a home health environments with a focus on safety and quality.
Collaborate effectively with physicians, therapists, social workers, and other healthcare professionals to coordinate treatment strategies and discharge planning.
Utilize electronic health records (EHR/EHR management) efficiently for documentation of medical records, progress notes, and case management activities.
Monitor patient progress through diagnostic evaluations and ongoing assessments; adjust care plans accordingly to optimize outcomes.
Educate patients and their families on health management strategies, medication adherence, infection control practices—including infectious disease care—and emergency procedures.
Ensure compliance with HIPAA regulations and health policy standards while maintaining accurate medical records and discharge documentation.
Participate in utilization management activities such as intake processing, managing cases as a nurse case manager, and supporting hospital-to-home transitions.
Provide specialized care for diverse populations including geriatrics, pediatrics and behavioral health patients.

Qualifications

Valid Registered Nurse (RN) license with current state certification.
Experience working within hospital medicine or outpatient clinics; familiarity with inpatient or post-acute care environments preferred, but willing to train new graduates.
Strong knowledge of health regulation policies including HIPAA compliance and medical terminology.
Proven ability in patient assessment, clinical case management, discharge planning, and managing electronic health records (EHR/EHR).
Excellent communication skills for client interaction and collaboration with multidisciplinary teams.
Ability to manage multiple cases efficiently while maintaining high standards of patient service.
Knowledge of physiology principles relevant to acute care patients; experience with diagnostic evaluation is advantageous.
Geriatrics expertise along with behavioral health or infectious disease experience is a plus.
