Full Job Description

Job Title: Licensed Occupational Therapist (OTR/L) | Weekend Premium Pay | Home Health

Compensation: $65 to $120 per Visit

Schedule: Every Weekend or Every Other Weekend Available

Location: Twin Cities Metro Area (21 counties broken up into territories)

Strong Clinical Support from an experienced interdisciplinary team that is available to answer questions, coordinate care, and help you succeed

What You'll Do

As a Weekend Occupational Therapist, you'll provide individualized therapy services that help patients safely return to daily activities while maximizing independence and quality of life.

Responsibilities include:

Complete comprehensive occupational therapy evaluations within the patient's home, assessing functional abilities, safety risks, cognitive needs, upper extremity function, and barriers to independence
Develop individualized plans of care based on the patient's medical condition, home environment, daily routines, personal goals, and physician orders
Provide skilled interventions that improve independence with bathing, dressing, grooming, toileting, meal preparation, medication routines, and other activities of daily living
Assess home safety and recommend practical modifications that reduce fall risks and make daily activities easier and safer
Train patients and caregivers on adaptive equipment, energy-conservation strategies, compensatory techniques, and safe ways to complete daily tasks
Address upper extremity strength, coordination, range of motion, fine motor skills, and functional use following illness, injury, hospitalization, or surgery
Help patients rebuild confidence and develop strategies that support independence within their actual living environment
Educate family members and caregivers on safe assistance techniques, environmental changes, and ways to support continued progress between visits
Collaborate with physical therapists, nurses, speech therapists, social workers, physicians, and other care team members to ensure services remain coordinated
Communicate meaningful changes in patient condition, safety, or functional status to the appropriate clinical team members
Complete accurate and timely electronic documentation that reflects the patient's needs, treatment provided, progress, and continued need for skilled services

You'll Be Successful If You Are

Compassionate and motivated by helping patients regain independence and remain safely in their homes
Comfortable working independently while maintaining consistent communication with the clinical team
An excellent communicator who can educate patients and caregivers in a clear, respectful, and encouraging way
Organized and self-motivated, with the ability to manage a flexible schedule and complete documentation on time
Skilled at evaluating the full picture, including the patient's physical abilities, cognition, routines, caregiver support, and home environment
Able to build trust quickly with patients and families during one-on-one home visits
Comfortable traveling between patient homes throughout an assigned area of the Twin Cities metro

Qualifications

Required

Current Minnesota Occupational Therapist license in good standing
Current CPR certification
Valid driver's license
Reliable transportation and the ability to travel between patient homes
Strong clinical assessment, communication, organization, and documentation skills
Ability to work independently and make appropriate clinical decisions in a home-based environment

Preferred

Previous home health, senior care, rehabilitation, skilled nursing, or geriatric occupational therapy experience
Familiarity with Medicare home health requirements and electronic medical record systems
Experience supporting patients with orthopedic, neurological, cognitive, functional, or chronic medical needs
Knowledge of adaptive equipment, durable medical equipment, fall prevention, and home safety recommendations
