Full Job Description

The Case Manager provides direct case management services to individuals and families who reside within the Boyertown Area Multi-Service (BAMS) coverage area. Duties include, but are not limited to, client intake and assessment, service planning, provision of information and referral, and program marketing and promotion. The Case Manager, working closely with the Director of Case Management, will help clients address social and economic barriers and work toward self-sufficiency, with a focus on rapid re-housing, prevention of homelessness, and overall improvement of quality of life.

Duties and Responsibilities

Conduct intake and assessment of clients and their overall situations.
Facilitate and develop individual action plans to improve clients' overall well-being based on information provided and professional assessment.
Provide clients with referrals for internal and external resources, including employment and appropriate social services.
Provide ongoing case management and follow-up when needed.
Maintain accurate client records, timely case notes, and documentation of client assistance requests.
Receive regular supervision and assistance in decision-making; demonstrate accountability for caseload and time management.
Prepare required case management reports and documentation to support program and grant compliance.
Produce correspondence and other documents as assigned.
Participate in the planning and execution of all BAMS holiday programs, including but not limited to the Easter Program, Back-to-School Program, and Christmas Program.
Assist with agency fundraising events as needed, including Preston's Pantry and the Festival of Trees.
Work effectively with team members on shared projects, including conferences, community development initiatives, grant proposals, and other agency activities.
Perform other duties as assigned.

Educational Requirements

Bachelor's Degree in Social Work, Human Services, or a related field.
Three (3) to five (5) years of social service program experience preferred.
Three (3) years of direct case management experience working with vulnerable populations preferred.
Must complete all required trainings, including but not limited to Mandated Reporter Training, Mental Health Training, and other relevant trainings as assigned.

Knowledge, Skills, and Abilities

Demonstrated ability to work independently and as part of a team.
Willingness to adhere to BAMS guidelines, policies, and procedures.
Understanding of, and commitment to, fair access to services for all clients and the ability to work effectively with a diverse population.
Ability to navigate stressful situations and manage competing priorities.
Strong client assessment skills.
Commitment to maintaining client confidentiality.
Excellent communication and organizational skills.
Must pass all applicable clearances, including FBI, Child Abuse, and Pennsylvania State Police background checks, on a yearly basis.
Ability to work effectively in both office and field settings.
Must possess a valid driver's license and maintain current vehicle insurance.

Physical Demands

In accordance with ADA requirements, employees are regularly required to sit, stand, walk, talk, and hear, both in person and by telephone.
Use of hands to operate standard office equipment is required; occasional reaching and lifting of up to 10 pounds may be required.
Ability to travel to various work and client locations is required.

Working Conditions

General office environment, home visits, and community-based settings.
Must be willing and able to travel throughout the BAMS service area and, on occasion, outside the service area.

Pay: $21.00 - $24.00 per hour
