Job Title: Clinical Pharmacist

Job Summary: The Clinical Pharmacist – PT, PD, CA is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of the Option Care, pharmacy practice professional standards, and applicable regulatory entities. This is an entry level position into the practice of clinical pharmacy in the alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals and educates patients, caregivers, staff and other professionals regarding appropriate medication use.

Key Responsibilities:

Patient Assessment: Evaluate and collect patient-specific information to determine their suitability for home care services, ensuring their medical condition and prescribed therapy align with home care goals.
Pharmacy Care Plans: Develop, monitor, and evaluate individualized pharmacy care plans for home care patients. Provide recommendations for modifications to therapy or monitoring as needed.
Patient Education: Educate patients, caregivers, and healthcare staff on medication use, utilizing effective communication and teaching techniques.
Medication Management: Prepare and dispense medications accurately, adhering to safety and quality assurance practices. Ensure compliance with federal and state laws, including DEA regulations.
Equipment Management: Maintain and troubleshoot medication administration equipment, providing guidance on its use and resolving any issues.
Drug Information: Provide timely, evidence-based responses to drug information requests from healthcare providers and patients.
Compliance: Ensure adherence to accreditation, legal, regulatory, and safety requirements while maintaining patient confidentiality.

Basic Qualifications:

Education: Graduate of an accredited School of Pharmacy.
Licensure: Active and unrestricted Pharmacy license granted by the State Board of Pharmacy.

Experience:

At least 1 year of experience in sterile IV admixture procedures, OR
Knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM), OR
Completion of an ASHP-accredited Pharmacy Residency Program, OR
Completion of an Advanced Pharmacy Practice Experience, OR
Equivalent experience approved by the VP of Pharmacy and/or SVP of Clinical Services.

Skills:

Customer Service: Experience delivering high-quality customer service and ensuring customer satisfaction.
Microsoft Office: Basic proficiency in Word, Excel, and PowerPoint.
Problem-Solving: Ability to apply critical thinking and problem-solving skills to resolve issues and improve patient care.

Travel Requirements:

Willingness to travel up to 5% of the time for business purposes within the state.

Preferred Qualifications:

Residency: Completion of an ASHP-accredited Pharmacy Residency Program or Advanced Pharmacy Practice Experience.
Experience: Previous experience in home care, hospital, or clean room settings.
Leadership: At least 1 year of experience in indirect management, including training and mentoring team members.
