Salary: $105k/yr - $110k/yr + Annual incentive up to $7,500 based on performance Depending on Experience

Job Description

The Clinical Care Manager provides clinical leadership, regulatory oversight, and hands-on field support for our non-medical home care services. This role completes and coordinates client assessments and recurring nursing visits, develops individualized and person-directed plans of care, builds and maintains the nursing visit schedule, supervises per diem nurses, and coaches caregivers in the field on care plan implementation, safety, and changes in client condition. This is not a back-office administrative role — the Clinical Care Manager maintains an active presence in clients' homes while owning clinical documentation in WellSky, compliance with New Jersey home care requirements and accreditation standards, and ongoing chart and care plan audits. This position works closely with ownership, scheduling, and operations while preserving clear boundaries between clinical decision-making and caregiver scheduling.

Education & Work Experience Requirements

Compact RN License or RN License in the state of New Jersey
2+ years of nursing leadership, supervisory, case management, or clinical operations experience
3+ years in private duty home care, personal care, or a comparable community care setting developing care plans, performing in-home assessments, supervising direct care workers, and coordinating field nursing assignments, recurring visits, and compliance-driven schedules
Strong understanding of the differences among private duty home care, skilled home health, hospice, facility nursing, and Medicaid-driven service models
Must be able and willing to travel throughout the agency's service area for client visits and Care Partner support
