Full Job Description

Fallon Tribal Health Center is seeking a compassionate and organized Case Manager / Patient Advocate to help patients navigate health care services, connect with needed resources, and coordinate care for patients who may be at risk for poor health outcomes. This position plays an important role in supporting patients’ physical, social, and emotional needs while promoting continuity of care across the Health Center and outside referral sources.

Key Responsibilities

The Case Manager / Patient Advocate will work directly with patients, families, health care providers, and community partners to support patient care and access to services. Duties include:

Assessing patients for case management needs, including care coordination, follow-up care, referrals, advocacy, and patient education.
Connecting patients to Health Center, tribal, and community resources, including housing, social services, public assistance, counseling, and other support programs.
Assisting uninsured patients and families with information about health insurance coverage and eligibility for available programs.
Coordinating with referral sources, family members, community agencies, and other care providers to support patient treatment and care plans.
Supporting elders and other vulnerable patients while they seek care at Fallon Tribal Health Center or through outside referral sources.
Helping locate resources for home care, durable medical equipment, transportation, skilled nursing, facility placement, and outpatient follow-up.
Developing service plans for patients needing case management services and making referrals consistent with those plans.
Documenting case notes, reports, patient concerns, complaints, progress notes, and related information in a timely manner.
Assisting with Advance Directive information and documentation when needed.
Working with Health Center leadership and staff to improve patient satisfaction and the overall patient experience.

Minimum Qualifications

Bachelor’s degree in Social Work, Counseling, Clinical Psychology, or a related field and at least four years of related work experience; or
Master’s Degree in Social Work (MSW).
Must obtain Case Management Certification, such as ACM, CCM, or ANCC, within 36 months of hire.

Knowledge, Skills, and Abilities

The ideal candidate will have:

Knowledge of community resources and case management principles.
Experience or familiarity with case management in a primary care or health care setting.
Strong communication skills and the ability to work with patients from diverse backgrounds.
Ability to maintain confidentiality, professionalism, and composure in stressful situations.
Ability to work cooperatively with patients, health center staff, tribal employees, community agencies, and the public.

Other Requirements

Must be able to pass required background checks.
Must complete Mandatory Reporter training within 90 days of hire.
Must possess a valid Nevada driver’s license, or be able to obtain one within 90 days, and remain insurable under the Tribe’s vehicle insurance policy.
This position is covered by applicable Indian Child Protection and Family Violence Prevention Act requirements.
