Physical Demands & Working Conditions:
Work is performed in a variety of settings such as clinics, schools, correctional sites, and homes. Activities are varied including patient care and similar work with moderate physical demands, as well as computer and telephone use, reading, and other work of a light nature. There is regular lifting, pushing or pulling such as when transferring patients, and moving supplies and equipment. There is considerable attention to detail. Regular driving is required for home visits and school coverage in all types of weather conditions.
Activities include stooping, kneeling, crouching, reaching, standing, walking, pushing, pulling, lifting, digital dexterity, grasping, feeling, talking, hearing/listening, seeing/observing, repetitive motions.
Medium Work: Physical demands are normally those associated with medium work: Exerting up to 50 pounds of force occasionally, up to 20 pounds of force frequently, and/or 10 pounds of force frequently or constantly.
The job may require dealing with individuals who are emotionally charged. There is regular exposure to potential hazards such as body fluids, contagious diseases and environmental hazards.

Special Assignment – School Nurse:
Assignments in addition to previously listed (1-10) essential duties and responsibilities

Maintains confidential health records for students at each school following both HIPPA and FERPA guidelines.

Administers medications based on doctor’s orders/standing orders as well as on an emergency basis, such as epi-pens.

Serves as a resource to school staff and parents.

Serves on school committees;

Conducts home visits as requested by school administration.

May plan and implement health fairs at schools.
Special Assignment – Maternal Child Health:

Provides professional nursing care through Maternal Child Health home visits.

Provides professional nursing care to Maternal Child Health clients as directed by a physician, community service agency or self-referral.

Provides professional nursing care through Nurse Family Partnership (NFP) visits.

Utilizes nursing skills to accomplish three NFP goals:

Improve families’ economic self-sufficiency.

Completes the necessary NFP education requirements for training and service provision;

Under the direct guidance and protocols of the Family Health Care Center, provides professional nursing care and perinatal coordination for pregnant women within the clinic setting.
Special Assignment – Immunization/Disease Control:

Plans and organizes immunization clinics throughout the community and oversees the operation of various clinics including flu and other immunizations following CDC recommendations and requirements.

Plans and organizes all tuberculosis control and international travel activities for the Public Health department.

Provides education and orientation on immunizations, TB and international travel for the general public, private and public healthcare providers, and others in the clinic or community setting.

Develops and delivers formal presentations, develops and obtains handout materials.

Actively participates in safety and wellness committees and assists departments with health promotion and wellness activities.

Collaborates with the Safety Coordinator on the Workers’ Compensation Program and OSHA requirements.

Assess, triage, counsel, and provide first aid to injured employees.

Special Assignment – Quality Assurance/HIPAA

Special Assignment – Women’s Way

Attend bi-monthly teleconferences and annual face-to-face coordinating unit meeting and/or any mandatory training required by the Women’s Way state office.

Provide community outreach to meet the minimum screening goal in the service area.

Recruit and enroll new women who meet the Women’s Way eligibility criteria.

Ensures all healthcare professionals, clinics, and facilities that submit claims for Women’s Way clients sign a Provider Cooperative Agreement before providing services.

Assist the State Office in ensuring all enrolled healthcare providers have the current Women’s Way “What’s Covered” list.

Remind healthcare providers to inform Women’s Way clients if they are going to perform a procedure not covered by the program.

Conduct at least six provider-clinic consultations annually in person.

Conduct community outreach, educate patients about risk factors and preventive health behaviors, identify eligible women for screening, and address barriers to care.

Navigate women to community resources, medical homes, or healthcare systems for cancer screening, diagnostic, and/or treatment resources.

Work with community partners to reach disparate populations.

Facilitate or refer to Medicaid Expansion or health insurance enrollment, if applicable.
