Full Job Description

JOB FUNCTION:

This position will provide initial screening, application assistance, client support, and referrals to households in need for a wide array of CAPLP programs including Head Start, homeless prevention, homeless services, and economic empowerment programs. Provide support to all Divisions and communicate with program community partners.

QUALIFICATIONS:

Minimum

One of the following
- Bachelor's degree in social work or a human service-related field
- Associates degree with 2 years of relevant experience
Valid driver’s license and current vehicle insurance

Preferred

Community Action experience
Experience working with families or individuals in crisis situations
Lived experience or a history working with people with low incomes

ESSENTIAL FUNCTIONS:

Greet and assist people that present to CAPLP offices in person, via phone, or by appointment to link to programs and services
Provide initial screenings and assist in collecting and completing documentation for families that may be eligible for CAPLP services
Provide intentional linkages for CAPLP services based on individual needs
Complete screening for public benefits such as SNAP, Medical Assistance, and CCAP
Develop rapport and build relationships with households to talk comfortably about their setbacks, accomplishments, and goals. Use a person-centered approach to assess individual and family strengths, develop plans, and coordinate needed services as well as recognize that people have the potential to grow and change
Provide referrals to community services as needed
Provide information on programs and answer questions to current or prospective clients
Track and report position data and outcomes to assess effectiveness and identify barriers
Complete intakes for CAPLPL programs
Engage in problem resolution, assess need/problem objectively, and manage crisis situations
Ability to be resourceful, flexible, adaptable, and demonstrate commitment
Communicate effectively both orally and in writing

Agency and Division Support

Orient and connect clients to program services and community resources
Represent the Agency in the communities we serve
Compile and submit required reports and data
Complete data entry needed for Agency programs
Assist with other program projects as requested by Divisions with CAPLP
Attend and participate in Agency, community, and state meetings important to effective Agency operations
Assess needs for professional development and conduct training for intra/interagency staff as required
Availability to work designated Agency hours for your Division and position
