Compensation: $95,000-$130,000 Annually + Incentive Visit Pay

Cover an established caseload while another RN takes vacation. Step into a territory during a leave of absence. Help a team through an unexpected absence. Support an area experiencing increased patient volume.

You Don't Need Three Weeks to Get Comfortable Somewhere New.

The right nurse for this position can walk into a changing situation and start connecting the dots.

Who are my patients?

Who needs to be seen first?

You'll have support from an established clinical team, but we're looking for an RN who brings enough experience and confidence to take ownership when stepping into someone else's territory.

That adaptability is a skill.

And in this position, it's one of your greatest assets.

You're Not Just Filling In. You're Keeping Care Moving.

When an RN takes PTO, their caseload doesn't disappear.

And we don't want our nurses feeling like they can't take a vacation because they're worried about what happens to their patients while they're gone.

That's where your role becomes especially important.

You'll provide continuity for patients while giving fellow clinicians the ability to actually step away from work when they need to.

You're not simply covering a hole in the schedule.

You're providing the clinical stability that allows the entire team to function better.

What You'll Do

Provide skilled nursing visits throughout Southwestern Minnesota.
Cover established territories during PTO, vacation, illness, and leaves of absence.
Step into changing caseloads and quickly identify patient priorities.
Complete comprehensive nursing assessments.
Provide admissions, routine visits, and case management coverage.
Develop, implement, and update individualized plans of care.
Recognize changes in condition and respond appropriately.
Educate patients and caregivers regarding medications, safety, and disease management.
Coordinate with physicians, therapists, social workers, and other clinical professionals.
Complete accurate and timely documentation, including OASIS when applicable.
Participate in rotating on-call responsibilities.
Help maintain continuity as clinical and territory needs change.

If you've already worked in home health, you understand what it takes.

You know how to walk into someone's home, complete a strong assessment, identify what's important, communicate with the provider, educate the patient, document appropriately, and manage your day without someone constantly directing your next move.

That experience can make you exceptionally valuable in a floating position.

Other field-based or complex clinical environments

Home health and OASIS experience are strongly preferred.

What We're Looking For

Active Minnesota Registered Nurse license.
Strong clinical assessment and independent decision-making skills.
Ability to quickly adapt to different patients and caseloads.
Willingness to travel throughout Southwestern Minnesota.
Excellent organization and prioritization.
Strong patient and caregiver education skills.
Confidence communicating with physicians and interdisciplinary teams.
Valid driver's license and reliable transportation.
Home health and OASIS experience strongly preferred.
