Capstone Health seeks Licensed Practical Nurse to join our team. The ideal candidate is service orientated and patient driven, enjoys working with underserved patients and is culturally sensitive. Provides nursing care in an ambulatory care setting, collects patient data, administers medications and treatments, and documents care provided in EMR system. Current LPN license in the State of Alabama required. Temporary Training Location: Please note that this position will ultimately be based at the Fayette, AL facility. Until the facility is fully open and operational, selected candidates will be required to complete training and/or perform assigned work at a Capstone Health location in Walker County, Alabama. Additional details regarding the training location, schedule, and transition to the designated facility will be communicated during the hiring and onboarding process.
PRIMARY ACCOUNTABILITIES:
- Improve patient outcomes and deliver quality care within practice areas as assigned. Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, and patient satisfaction.
- Ensures timely and accurate collection of patient data and makes observations on assigned patients to provide supportive data in the patient assessment.
- Promotes attainment of specified goals through health education to patients/families, and healthcare personnel as delegated by the registered nurse.
- Provides service in a manner that is appropriate for the patient’s age; demonstrates knowledge and skills necessary to meet the patient’s physical, psychosocial, educational, and safety needs.
- Ensure all patient charts and related documentation are maintained current and relevant at all times.
Operational Excellence
- Ensure all care and nursing practices are conducted consistent within current, relevant standards, as well as state and local laws and regulations, JCAHO and/or PCMH accreditation and related professional standards.
2. Ensure and uphold the confidentially requirements of all patient data and manage all daily task and activities consistent with HIPAA, state and federal laws and regulations, as well as the clinic’s policies and regulations regarding confidentiality and security.
- Demonstrate competency in practice and knowledge of current standards of practice. Maintains credentials and performs current nursing practice expectations within established guidelines.

PRIMARY TASKS & DUTIES:
- Provide direct care to patients as assigned and qualified to perform.
- Assess, review, and develop nursing plans to ensure maximum patient outcomes.
- Provide counsel, support, advice, and support to patients and their families.
- Complete all patient charting and ensure all patient records are current and complete.
- Attend meetings, patient conferences, planning sessions, related to quality assurance, patient care, and other related topics within the clinic.
- Attend seminars and maintain all licensure requirements for continuing education and best practices.
- Participates in quality strategies to evaluate compliance with standards and to identify opportunities to improve patient outcomes.
- Mentors and assists students performing clinical rotations.
- Other duties as assigned.

POSITION REQUIREMENTS:
Education
- Graduation from an accredited practical or vocational nursing program. New graduates must take the state board exam within 90 days of graduation date from nursing program. Certified BCLS is required.
Experience
- Demonstrated success in customer service or working with the general public, preferably in a medical care facility.
- Demonstrated success in managing difficult customer situations
- Significant knowledge of medical practices and procedures in a primary care environment.
- Demonstrated success and experience with direct patient services (e.g., phlebotomy) and the proper knowledge and use of medical devices and equipment.
Physical/ Environmental
· Occasional lifting of 40 – 50lbs. and pushing of 5-20lbs.
· Sitting, standing and walking approximately 80% of the time
· A medium to high level of manual dexterity required
· Bending and reaching approximately 20% of the time
Clinical working environment. Requires good verbal and written communication skills. Must be able to speak and read the English language.
Ability to move freely (standing, stooping, walking, bending, pushing and pulling) and lift up to a maximum of fifty (50) pounds without assistance.
Work is primarily facility based in a primary care facility.
Some work may occur in a community setting and may include contacts in a high crime area and in extremes of temperature.
