Full Job Description

Fieldstone Nursing and Rehabilitation Center is looking to hire a Licensed Nursing Home Administrator (LNHA) to join our team. If you are energetic, upbeat, and love making a difference in residents' and patients' lives, this is a great fit. Fieldstone is nestled away in a beautiful area of Fostoria, Ohio, and is a 64-bed skilled nursing and rehabilitation facility.

Duties and Responsibilities:

Responsible for the leadership, oversight, and daily operations of the facility.
Oversee day-to-day operations of all facility departments and services, and ensure that compliance is ongoing.
Development and implementation of a facility management system.
Monitoring and maintaining the facility's compliance with all pertinent state and federal regulations, including Ohio Department of Health and CMS requirements.
Monitor monthly performance of the facility against budget and intervene as needed.
Lead census development and maintain hospital, physician, and managed care referral relationships.
Recruit, develop, and retain department heads and facility staff.
Makes sure the facility is a safe, clean, comfortable, and appealing environment for residents, patients, families, and staff according to company guidelines.
Complete required forms and documents following company policy and state and federal regulations.
Manage all aspects of the state and federal government survey process, including plan of correction and QAPI.

Qualifications and Skills:

Active Licensed Nursing Home Administrator license in Ohio (BELTSS)
Skilled nursing or long-term care administrator experience preferred, and recent AIT graduates are encouraged to apply
Must be highly organized, professional, and motivated
Excellent written and verbal communication skills
Knowledge of Medicare and Medicaid procedures and applicable policies
Extensive knowledge of current federal, state, local, and corporate guidelines and regulations governing management in long-term healthcare facilities
Reports to the Governing Body
