Full Job Description

DUTIES AND RESPONSIBILITIES:

Develops a POC/485 that corresponds with the physician orders through assessment of patient's needs, condition, environment, and consultation with other health team members at the time of the initial visit. Updates the POC regular, at least every 60ndays, and as necessary. Document this information following Life's Touch Home Health policies / procedures.

Responsible for ensuring the delivery of quality patient care in the home in accordance with best standards of practice.

Maintains the ongoing responsibility for assigned case loads.

Responsible for the implementation and evaluation of individual patient POC's and make changes as needed.

Maintains communication with other Case Managers, Staffing Coordinator, and Clinical Directors of Nursing, and Directors of Nursing in a timely manner.

Provides teaching education and counsels both the best practices, agency policies and procedures.

Rotates weekend , night and holiday on call responsibilities with other Case Managers to facilitate appropriate coverage for all patients in an emergency situation.

Participates in Life's Touch Home Health evaluation programs as instructed.

Regularly conducts case conferences in accordance with regulations and policy guidelines.

Abides by and promotes Life's Touch Home Health administrative policies and procedures.

Responsible for producing weekly and monthly reports pertinent for maintaining quality assurance.

Weekly nursing meetings and in services.

Required to manage and train nursing staff.

EDUCAION AND QUALIFICATIONS

Registered Nurse Licensed in accordance with the laws of the State of Indiana with no restrictions.

Minimum of 1 year experience as a RN, and preferably home care experience.

Strong communication skills and ability to collaborate within a team environment.

Has previous experience in a nursing leadership role, and in depth knowledge of the healthcare industry.

KNOWLEDGE , SKILLS AND ABILITIES REQUIRED:

Ability to communicate technical information in a clear and understandable manner.

Ability to develop, implement and modify multidisciplinary health care plans.

Ability to make administrative/procedural decisions and judgments in regards to patient care and safety.

Knowledge of billing procedures for clinical services

Lead the nursing staff in delivering excellent patient care and offering quality support to the medical staff.

Ensures that each nurse on staff is properly certified and abides by the standard of care established by our organization and State and Federal regulations.

Create new policies/ procedures and update existing policies/procedures to help improve the level of care for each patient.

Implement and evaluate quality improvement programs designed to help improve the level of care for each patient and to help us deliver the highest possible level of service.

Ensures that all patient care plans have the documentation necessary to provide the proper type of care per Physician orders and client preferences.

Knowledge of clinical documentation procedures and standards.

Knowledge of nursing and case management theory and practice.

Knowledge of patient care, charts, and patient case history.

Organizing and coordinating skills.

Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community while respecting cultural preferences and choices.
