Full Job Description

Compensation Range: $65,000 - $70,000/year commensurate with experience

Benefits:

Overview:

The Program Manager is responsible for the overall management, implementation, and quality oversight of the AI PrEP Improving Equity and Hepatitis C Care and Treatment Programs across Housing Works' Brooklyn and Manhattan health centers.

This role oversees the day-to-day operations of both programs, ensuring the delivery of high-quality, client-centered services that improve access to HIV prevention, Hepatitis C screening, linkage to care, treatment, and retention in care for priority populations.

The Program Manager provides direct supervision and support to Prevention Navigators, Outreach Specialists, and other assigned program staff across multiple clinic sites. The position is responsible for monitoring staff performance, managing caseloads and workflows, ensuring timely documentation and reporting, and maintaining compliance with grant requirements, agency policies, and clinical protocols.

Working closely with the Program Director, the Program Manager leads quality improvement initiatives, monitors program performance metrics, identifies opportunities for service enhancement, and develops strategies to improve health equity, client engagement, and overall program outcomes.

This position serves as a key operational leader in the expansion of biomedical HIV prevention services, including oral and long-acting injectable PrEP, while also overseeing comprehensive Hepatitis C care and treatment programming. The Program Manager collaborates with medical providers, nursing staff, pharmacists, data teams, referral coordinators, and community partners to ensure seamless service delivery from outreach and enrollment through long-term retention in care.

Using a multidisciplinary, status-neutral approach within both primary care and community-based settings, the Program Manager works with diverse populations, including:

Individuals experiencing housing instability or homelessness
People who use drugs
Individuals living with mental health conditions
LGBTQ+ communities
Immigrants
Other populations disproportionately impacted by HIV, Hepatitis C, and social and structural determinants of health

The Program Manager also works collaboratively across Housing Works programs—including Primary Care, Prevention Services, Behavioral Health, Substance Use Treatment, Youth Services, and other supportive service programs—to strengthen referral pathways, address barriers to care, and improve client health outcomes.

Job Requirements:

Bachelor’s Degree preferred Experience with program management.
Knowledge and experience with the dually diagnosed community of HIV/AIDS and mental illness.
Knowledge and experience with state and local policies of NYC affecting persons living with mental illness, HIV/AIDS. Some
Experience in program planning and program operations.
Experience working with homeless populations and active substance users.
