Full Job Description

Clinical Liaison & Patient Care Coordinator

Location: McLeod Health – South Carolina
Company: National Diabetes Care (NDC)

National Diabetes Care (NDC) partners with physicians, hospitals, and healthcare systems to provide diabetic footwear, custom inserts, compression garments, and other therapeutic products that help improve patient outcomes.

We are seeking a Clinical Liaison & Patient Care Coordinator to serve as our onsite representative at McLeod Health. This position combines patient care coordination, physician relationship management, and program development in a rewarding healthcare environment.

Responsibilities

Serve as NDC's onsite representative within McLeod Health.
Build relationships with physicians, nurses, podiatrists, endocrinologists, wound care teams, and hospital staff.
Identify and assist patients who qualify for diabetic footwear and related products.
Educate providers on NDC programs and services.
Collect patient information, insurance, and medical documentation.
Coordinate orders and provide excellent follow-up with patients and providers.
Support affiliated physician offices, outreach clinics, and occasional home visits, as needed.
Help grow NDC's presence through outstanding service and provider engagement.

Qualifications

Healthcare, medical office, DME, insurance, or patient care experience preferred.
Strong customer service, communication, and organizational skills.
Experience with EMRs and healthcare documentation.
Medical Assistant, CNA, LPN, RN, Pedorthic, DME, wound care, podiatry, or endocrinology experience is a plus.

Schedule & Compensation

Flexible weekday schedule (position begins on a part-time basis).
Starting at $18/hour, with higher pay based on experience and qualifications.
Performance incentives available.
