POSITION DESCRIPTION
TITLE: Family and Health Services Coordinator
STATUS: Full-Time, Hourly
DEPARTMENT: Head Start / Early Head Start
REPORTS TO: Program Manager
PRIMARY FUNCTION
The Family and Health Services Coordinator is responsible for coordinating comprehensive family engagement and health services for enrolled Head Start and Early Head Start children and families. This position serves as a liaison between families, program staff, healthcare providers, community agencies, and other service organizations to promote healthy living, family well-being, school readiness, and positive child outcomes.
The Family and Health Services Coordinator utilizes a family-centered, strengths-based approach to support families in identifying goals, accessing resources, overcoming barriers, and advocating for their children. The position is also responsible for coordinating and monitoring health services to ensure compliance with Head Start Program Performance Standards and applicable federal, state, and local requirements.
KEY ROLES (Essential Job Responsibilities)
Family Services
- Develop and maintain positive, professional, and supportive relationships with enrolled families.
- Implement the family partnership process, including family strengths assessments, goal development, and ongoing progress monitoring.
- Assist families in identifying strengths, needs, and resources that support family well-being and child development.
- Conduct home visits, family conferences, and follow-up contacts in accordance with program requirements.
- Facilitate family engagement opportunities and encourage active participation in program activities.
- Assist families in accessing community resources including housing, transportation, healthcare, employment, education, childcare, food assistance, and other support services.
- Support families in developing skills that promote school readiness and positive child outcomes.
- Encourage and facilitate parent participation in Parent Committee and Policy Council activities.
- Promote family leadership, advocacy, and self-sufficiency.
- Assist families of children with disabilities and collaborate with Disabilities Services staff as needed.
- Recognize and respect the diverse cultures, values, and backgrounds of families served.
- Establish and maintain professional boundaries while building collaborative partnerships with families.
Health Services
- Coordinate and monitor all child health requirements including physical examinations, immunizations, dental examinations, developmental screenings, hearing screenings, vision screenings, and follow-up treatment.
- Ensure compliance with Head Start health requirements and applicable pediatric and dental periodicity schedules.
- Maintain accurate and current health records for all enrolled children.
- Monitor health compliance data and identify children requiring follow-up services.
- Coordinate referrals and follow-up treatment for medical, dental, developmental, behavioral health, and mental health concerns.
- Develop, maintain, and monitor Individual Health Plans (IHPs) for children with identified health conditions.
- Collaborate with healthcare providers, specialists, families, and staff to support children with ongoing health needs.
- Coordinate health education activities and resources for children, families, and staff.
- Facilitate and coordinate Health Services Advisory Committee (HSAC) meetings.
- Support health and safety initiatives and continuous quality improvement efforts.
Community Partnerships
- Develop and maintain collaborative relationships with community agencies, healthcare providers, service organizations, and other community partners.
- Coordinate referrals and follow-up services through community resources.
- Maintain current information regarding community services available to families.
- Participate in community planning activities and collaborative partnerships that support children and families.
- Represent the program at community meetings and events as assigned.
Documentation and Compliance
- Maintain complete, accurate, and confidential family and health records.
- Complete all required documentation and data entry accurately and within established timelines.
- Ensure compliance with Head Start Program Performance Standards, component plans, agency policies, and applicable regulations.
- Assist with program monitoring, audits, self-assessments, and federal reviews.
- Prepare reports and maintain records necessary for program operations and compliance.
- Assist with collection and documentation of non-federal share (in-kind) contributions as assigned.
- Maintain confidentiality regarding children, families, staff, and agency information.
Professional Responsibilities
- Demonstrate agency values and maintain positive relationships with children, families, staff, volunteers, and community partners.
- Attend required staff meetings, trainings, conferences, and professional development activities.
- Maintain current knowledge of Head Start regulations, family engagement practices, and health services requirements.
- Participate in activities designed to ensure program quality and continuous improvement.
- Maintain CPR and First Aid certification as required.
- Report suspected child abuse or neglect in accordance with agency policy and state law.
- Perform other duties as assigned to support program operations.
RELATIONSHIPS
Internal
Maintains regular contact with Program Management, Education Staff, Disabilities Staff, Nutrition Staff, Mental Health Consultants, Health Providers, Administrative Staff, and Volunteers to ensure coordinated service delivery and program success.
External
Maintains regular contact with families, healthcare providers, dentists, specialists, community agencies, volunteers, consultants, and service organizations to support family engagement and health outcomes.
SKILLS / KNOWLEDGE / QUALIFICATIONS REQUIRED
Minimum Qualifications
- High School Diploma or GED.
- Minimum three (3) years of experience in Head Start, Early Head Start, family services, health services, social services, case management, early childhood education, healthcare administration, community outreach, or a related field.
- Valid Florida Driver’s License and insurable driving record.
- Strong interpersonal, communication, organizational, and problem-solving skills.
- Ability to maintain confidentiality and professional boundaries.
- Ability to work independently and manage multiple priorities.
- Proficiency in Microsoft Office and electronic data management systems.
Preferred Qualifications
- Family Services Credential.
- Associate’s Degree or Bachelor’s Degree in Human Services, Social Work, Public Health, Health Education, Psychology, Sociology, Family Studies, Early Childhood Education, Nursing, Business Administration, Public Administration, or a related field.
- Previous Head Start or Early Head Start experience.
- Experience coordinating family engagement services, health services, community partnerships, and case management activities.
Professional Development Requirement
Employees hired without a Family Services Credential must obtain the credential within twelve (12) months of hire as a condition of continued employment unless an extension is approved by the Chief Executive Officer due to training availability or other documented business necessity.
PHYSICAL REQUIREMENTS / WORK ENVIRONMENT
- Physically able to perform duties which may include lifting up to 25 pounds, bending, reaching, stooping, and moving throughout classrooms, offices, homes, and community settings.
- Maintain mental and physical alertness necessary to perform essential job duties.
- Work may be performed indoors and outdoors and may involve interaction with young children and families.
- Travel throughout the service area may be required for home visits, meetings, training, and community events.
- Occasional evening and weekend work may be required for parent meetings, family engagement activities, community events, and professional development.
- Ability to operate computers, tablets, telephones, and other office equipment necessary to perform assigned duties.
- Noise level may vary from moderate to loud depending on work location and activity.
ADDITIONAL REQUIREMENTS
- DCF Level II Background Screening clearance required prior to employment and throughout employment.
- Physical examination and TB screening required as a condition of employment.
- Must comply with all agency policies, procedures, and Head Start Program Performance Standards.
- Must maintain all required certifications and training.
