At First Light Home Care, we’re dedicated to delivering compassionate, high-quality home health care that helps patients recover safely in the comfort of their homes. We’re expanding our reach and are seeking an experienced Community Liaison to join our team and build strong relationships within the healthcare community.

Position Summary

The Home Health Community Liaison will be responsible for developing, nurturing, and maintaining referral relationships with hospitals, physicians, case managers, and other healthcare professionals. This role focuses on driving growth through education, outreach, and exceptional customer service.

Key Responsibilities

Develop and implement strategies to increase patient referrals and market share.
Build and maintain strong relationships with referral sources.
Educate healthcare professionals about our services and programs.
Conduct presentations and community outreach activities.
Track sales performance and market trends to identify opportunities.
Collaborate with clinical and administrative teams to ensure client satisfaction.
Meet or exceed monthly and quarterly referral goals.

Qualifications

Bachelor’s degree in Marketing, Business, Healthcare Administration, or related field (preferred).
Minimum of 2 years’ experience in home health, hospice, or healthcare sales.
Proven ability to meet and exceed sales goals.
Excellent communication, presentation, and relationship-building skills.
Valid driver’s license and reliable transportation.
