ROLE SUMMARY____________________________________________________________

Life is looking for a detail-oriented, metric-driven, and mission-aligned Eligibility Enrollment Specialist to support patients through the approval process for care. This role is responsible for navigating all managed long term care eligibility requirements—removing regulatory and documentation barriers so qualified patients can move forward to admission.

The Eligibility Enrollment Specialist owns the Eligibility & Enrollment stage of the patient journey, working closely with Patient Advocates and Intake to support qualified patients through the eligibility and enrollment process You will guide patients and families through the New York Independent Assessor (NYIA) process and Uniform Assessment System (UAS), ensuring accuracy, responsiveness, and a smooth enrollment experience .

This role requires strong ownership, attention to detail, and the ability to manage multiple patient cases with care, urgency, and accuracy. You must be highly organized, proactive in resolving barriers, and committed to helping patients and families move smoothly through the approval process. This is also a metrics- and progress-driven role, where success is measured by timely case movement, responsiveness, and timely enrollment outcomes.

WHAT YOU’LL NEED TO DO_____________________________________________

Eligibility & Enrollment Case Management (80%)

Develop deep expertise in NYIA policies and assessment requirements, proactively monitoring policy updates from official sources and ensuring changes are effectively implemented across day-to-day enrollment operations
Own the end-to-end eligibility and enrollment process for each assigned case, supporting patients through the approval process for care
Resolve eligibility barriers to ensure patients receive appropriate care:
NYIA process coordination
UAS assessment preparation and readiness
Ensure all required patient documentation is complete, accurate, and submitted in a timely manner
Proactively identify and resolve blockers (missing documents, eligibility gaps, scheduling delays, etc.)
Coordinate closely with Patient Advocates and Intake to ensure qualified, engaged cases enter the enrollment process

Activity, Reporting, and Team Collaboration (15%)

Own visibility into all cases within the eligibility and enrollment pipeline
Track progress across Medicaid, NYIA, and UAS stages, minimizing delays and reducing case drop-off risk
Participate in weekly pipeline reviews with Patient Advocates and Intake, identifying blockers and driving next steps
Maintain accurate and up-to-date case tracking in CRM systems (e.g., Salesforce, Google Sheets)

Other Duties as Assigned (5%)

QUALIFICATIONS & EXPERIENCE_________________________________________

6+ year experience in healthcare operations, case management, or a process-driven service role (homecare, or New York Independent Assessor experience strongly preferred)
Strong understanding (or willingness to learn) Medicaid, NYIA, and UAS processes
Proven ability to manage complex workflows with multiple steps and stakeholders
High attention to detail and accuracy in documentation and process execution
Experience tracking workflows and managing case progress in CRM tools (Salesforce, Google Sheets)
Ability to work cross-functionally and coordinate effectively with multiple teams
Strong communication skills, especially in guiding patients and families through structured processes
Fluent in English and Mandarin (required); Cantonese or Fujinese a strong plus
Willingness to work in a fast-paced environment with clear performance targets

LOCATION____________________________________________________________

Flushing, New York (~30–45 minutes from Manhattan). This person is expected to work in-office 4–5 days per week, conduct field/home visits as needed, and travel throughout Queens and Long Island as needed to support patient enrollment activities.

Flexibility to support patients during evenings or weekends when required is expected.

Pay: $66,300.00 - $70,000.00 per year
