Full Job Description

JOB TITLE: PATIENT ACCESS SPECIALIST (Onsite in our office)

REPORTS TO: Patient Access Manager

JOB SUMMARY: To support seamless patient transition into agency’s services by interfacing with hospital/physicians, and families to coordinate and process patient referrals.

RESPONSIBILITIES:

Acts as the primary in-office customer service contact in the Patient Access Center via telephone with referrals for patient care from physicians, hospitals, insurance case managers and other sources in the community.
Responds to communication regarding potential referrals for services with an extremely customer friendly and professional service orientation at all times.
Collects basic information to accurately complete referral documentation as necessary to facilitate the admissions process.
Initiates documentation on referrals and enters information into system to support Patient Access Manager.
Under the guidance of the Patient Access Manager, serves as a liaison between referral sources and agency regarding referral of patients who need direct admission or who need home care services upon discharge from the hospital.
Contacts patient/family as soon as possible after referral is received.
Works closely with management, Business Development team, and peers to support the day-to-day operations of the team.
Works closely with Patient Access Manager to facilitate discussion about referrals during the referral-to-admission process.
Understands that teamwork is a crucial part of our business, and is able to work well with colleagues, no matter the circumstances. Using individual skills, cooperates with others, and accepts and provides constructive feedback despite any personal conflicts between individuals involved.
Processes and documents referrals that are not accepted for admission, indicating specific reason for non-acceptance.
Utilizes current agency and/or department-specific software to complete assignments.

MINIMUM QUALIFICATIONS:

Must have exceptional interpersonal and customer services skills via telephonic communications.
MUST BE FULLY COVID-19 VACCINATED.
Must have knowledge of basic medical terminology.
Must have high school diploma or equivalent with successfully completed course work.
Must have one (1) year of experience working in a healthcare environment. Clerical and computer skills required, including data entry, office machines, and working knowledge of phone systems.

ADDITIONAL DESIRABLE QUALIFICATIONS

Bilingual - Spanish speaking
Must be proficient in the use of Microsoft programs for internal communications (email) and database tracking/reporting, as well as proficiency working with electronic health records.
Committed to the Hospice philosophy.
Must have excellent oral and written communication skills and a strong commitment to customer service.
