Full Job Description

Job description:

Healthspan Partners (HSP) is seeking an autonomous senior services professional to serve as a Dementia GUIDE Care Navigator. HSP is a remote, comprehensive dementia care model founded on the Medicare GUIDE (Guiding an Improved Dementia Experience) Dementia Program.

This family-centered role focuses on supporting people living with dementia who reside in private residences, along with their dedicated caregivers. The Care Navigator will introduce families to the GUIDE benefit, provide vital caregiver education and training, and seamlessly connect families to community-based services and supports.

Key Responsibilities

Caregiver Support & Training: Provide remote coaching, education, evidence-based training, and support to family caregivers navigating the daily challenges of dementia.
Community Resource Connection: Act as a bridge to community-based services and supports, helping families access local respite care, adult day programs, meals, and social services.
GUIDE Benefit Introduction: Educate assigned families and community-based organizations on the features, resources, and mechanics of the Medicare GUIDE benefit.

Critical Qualifications

Dementia Care Experience: A minimum of 5 years of experience supporting people living with dementia and their caregivers in a community or home-based setting, with a deep dedication to this population.
Geographic Requirement: Must currently reside in the state of Florida.
Relevant Professional Background: Experience in at least one of the following areas:
Social Work (SW) or Family Case Management
Community-Based Organizations (CBOs) serving high-needs older adults
Case Management, Discharge Planning, Transitions of Care Specialist

Valued (But Not Required) Attributes

Community & Sector Connections: Deep understanding of, and existing professional connections to, senior services sector and community-based programs.
Model Familiarity: Familiarity with the GUIDE Model Care Navigator (CN) framework.
Credentials: Active Social Work (SW), LPN, RN, or OT certifications are valued but not required.

Pay: $35.00 - $50.00 per hour
