Full Job Description

The primary function of this position is to collaborate with participants and / or designated representatives to identify and obtain resources that address gaps in the care plan prepared by the IDT team, such as, community resources to meet the social and/or emotional needs that affect the participants’ ability to achieve their goals and assist in acquisition of social services in compliance with PACE, federal, state, and local regulations.

Essential Duties/Responsibilities

· Demonstrates critical thinking skills in discriminating between protection of sensitive medical/psychosocial health information in compliance with applicable law and regulation regarding mandated reporting.

· Demonstrates the ability to access various community resources, such as: food, housing, and utility assistance, and benefit acquisition for state and government level.

· Identifies existing problems, utilizing solving skills in collaboration with participant/designated representative based on plan of care, assessment, and the availability of support systems within the community.

· Initiates referrals to appropriate community resources and assists participant and designated representative in resource acquisition; identifying steps and possible barriers to acquisition of resources.

· Accesses consultation with MSW members of the Interdisciplinary Team (IDT) during team meetings regarding participant care and/or within the supervision process.

· Documents care provided and the response of the client/family in a risk-neutral fashion; recognizes the sensitivity of psychosocial data and protects confidentiality of same; submits documentation which reflects third-party payor requirements in a timely fashion as described in agency policy.

· Assist with collaboration with applicable PACE departments/programs on the acquisition of materials in preferred language for participant care.

· Completion of participant education which includes annual acknowledgement of services received, participant rights, grievance process within regulations established for benefits received (i.e.. Medicare and/or Medicaid).

· Participate in community meetings to stay abreast of resources available and process for applying for assistance focused on participant care (i.e. housing authority, utilities assistance, etc.)

Required Education and Experience

Bachelor’s degree from four-year college or university.

Preferred Education and Experience

Social work bachelor’s degree requirement from four-year college or university. Two years full-time, direct service experience with the elderly or disabled (includes assessment, care plan development and monitoring).
