Full Job Description

Summary/Objective

Performs all activities per intervention protocol to achieve program goals. The Respite Coach will provide non-medical respite and coaching services to caregivers of any age, throughout the counties of Allen, Adams, DeKalb, Huntington, Steuben, LaGrange, Noble, Wells, and Whitley. Respite Coaching includes education on Dementia care, providing caregivers with support as they learn to help support their loved ones with the ability to keep them in the community and out of acute care. The Respite Coach supports the wishes of the client and caregiver to remain safely in the community and their homes.

Essential Duties/Responsibilities

Complete Aging Brain Center (ABC) care coaching certification.
Develop and maintain expertise on evidence-based dementia care and effective ability to train caregivers in same.
Is aware of and adheres to all elements of the individual service plan.
Will be working directly with persons living with dementia, orchestrating memory enhancing activities, and encouraging social interaction.
Provide required documentation in a precise, accurate, and timely manner to meet required deadlines upon each visit with a client or family member using program specific software program, i.e.: Weekly Activity Logs, Client Reports, Mileage Log, etc.
Attend trainings and meetings as requested by supervisor.
Maintain a working knowledge of the changing needs of senior and disabled populations.
Support caregivers in learning how to access community care.
Support client and caregivers in creating connections with the local community.
Develop and maintain accurate knowledge of, and monitor compliance with, HIPAA and agency confidentiality policies.
Serve as an example of agency Standard of Excellence by exemplifying agency vision and goals.
Communicate with supervisor on daily basis, and as needed, to report all areas of concern/issues/incidents.
Demonstrate professionalism in relationships with customer/co-workers/supervisors to build rapport while refraining from inappropriate familiar personal relationships.
Complete and attest to accuracy of payroll records and submit for review and processing according to agency schedule.

REQUIRED EDUCATION AND EXPERIENCE

·High School Diploma/GED required.
Two years working with or exposure to clients living with chronic medical conditions and/or chronic cognitive decline (Dementia or related disease).

PREFERRED EDUCATION AND EXPERIENCE

·Associate degree, and two years of related experience preferred.
Experience working with older adults preferred.
Prior experience in a healthcare environment preferred.
