NURSE CLIENT ADVOCATE
Pay: $38.00-$45.00 per hour

POSITION OVERVIEW
Under the supervision of the Practice Lead, the Nurse Client Advocate (NCA) coordinates all aspects of a client’s needs, including medical, environmental, psychosocial, financial, and legal considerations. The goal of this role is to improve each client’s health, well-being, and overall quality of life.
The Nurse Client Advocate is a trained and licensed Registered Nurse who “Guides the Way” for PyxisCare clients in their pursuit of exceptional health and healthcare. The role combines clinical expertise with ongoing client relationships, advocacy, communication, availability, information management, and resource coordination.
PyxisCare does not provide traditional bedside nursing services. Instead, we take a consultative, multidisciplinary approach to supporting our clients. Our clients represent a wide range of ages, health conditions, socioeconomic backgrounds, and geographic locations. A successful Nurse Client Advocate must be able to communicate effectively, adapt quickly, and build trusted relationships with clients, families, providers, and other stakeholders.
WORK SCHEDULE AND FLEXIBILITY
The majority of work is performed between 8:00 a.m. and 5:00 p.m., Monday through Friday. However, client appointments, healthcare needs, and unexpected circumstances may extend beyond regular business hours or occur on weekends or holidays. Nurse Client Advocates are expected to take ownership of their assigned cases and respond to client needs when circumstances require attention outside the standard schedule.
This is a full-time position requiring significant flexibility and the ability to adjust or pivot schedules based on client needs, appointments, emergencies, and other case-related priorities.
Most working hours are typically spent attending client visits, provider appointments, meetings, and other field-based activities. The position also includes remote work for client communication, research, care coordination, documentation, reporting, and other administrative responsibilities.
Regular daytime travel is expected, and some overnight travel is required.

ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties are fundamental to this position. Additional responsibilities may be assigned at the discretion of PyxisCare Management.
- Assess: Conduct comprehensive, multidisciplinary health and healthcare assessments in the field or virtually. Assessments must incorporate all relevant aspects of the client’s situation and follow PyxisCare Management forms, screening tools, and protocols.
- Plan: Collaborate with the Practice Lead to develop an individualized plan of care addressing the client’s identified needs, current medical status, goals, and circumstances.
- Communicate: Prepare detailed assessment reports that summarize findings, identified concerns, and recommendations. Communicate clearly and consistently with clients, families, healthcare providers, and other stakeholders involved in the client’s care.
- Implement: Attend provider appointments, meetings, conferences, and other engagements as required by the needs of the client and the parameters established by PyxisCare Management policies and protocols.
- Coordinate and Advocate: Help clients navigate healthcare services, treatment options, community resources, benefits, and other systems affecting their health and well-being. Advocate for the client’s needs and assist with resolving barriers to care.
- Maintain Case Ownership: Take responsibility for assigned cases, monitor client needs, follow through on commitments, and respond appropriately when circumstances require attention outside standard business hours, including weekends and holidays.
- Adapt to Client Needs: Maintain a flexible schedule and be prepared to adjust planned activities when appointments, urgent concerns, or changing client needs require a shift in priorities.
- Document: Maintain accurate and complete records of case activities, outcomes, billable hours, expenses, and client-related communications. Provide case updates, summaries, and strategic plans when requested. All documentation must be accurate, complete, and submitted on time.
- Complete Remote Responsibilities: Perform necessary remote work, including documentation, research, client and provider communication, appointment coordination, follow-up activities, and case planning.
- Perform other duties as assigned.

EDUCATION AND EXPERIENCE
- Associate Degree in Nursing, Bachelor of Science in Nursing, or a higher nursing degree from an accredited nursing program.
- Five or more years of clinical nursing experience.
- Home health experience is preferred.
- Case management experience is preferred.
REQUIRED LICENSURE
- Current, active Registered Nurse license.
- Valid driver’s license.
- Reliable transportation and the ability to travel regularly during the workday.
- Ability and willingness to complete occasional overnight travel.

WORK LOCATION
This is a hybrid, remote, and field-based position serving clients throughout the Dallas-Fort Worth area. Regular local and regional travel is required, with occasional overnight travel.
Compensation is commensurate with experience.
