Job Summary

We are seeking an experienced Clinical Pharmacist to join our healthcare team. The ideal candidate will have a strong background in patient care and be committed to providing exceptional pharmaceutical services. This role is responsible for ensuring the safe and effective use of medications in a hospital setting, and we are looking for a dedicated professional who can make a positive impact on our patients' health outcomes.

Duties

- Provide expert advice to medical staff and patients on the use of medications, including pain management options.
- Conduct patient assessments and develop individualized medication plans.
- Participate in interdisciplinary rounds and contribute to patient care decisions.
- Ensure that all medications are dispensed and administered according to hospital policies and procedures.
- Maintain accurate and up-to-date records of patient medications and treatment plans.
- Collaborate with other healthcare professionals to develop and implement clinical pharmacy services.
- Develop and implement policies and procedures related to pharmacy operations.
- Stay current with the latest developments in pharmacy practice and medical terminology.
- Provide education and training to other healthcare staff on pharmacy-related topics.

Skills

- Completion of a pharmacy residency program or post-residency experience.
- Strong knowledge of medical terminology and aseptic technique.
- Experience working in a hospital setting, including the ER and ICU.
- Proven experience in patient care and patient assessment.
- Ability to effectively communicate with patients and healthcare professionals.
- Familiarity with pain management options and medications commonly used in hospital settings.
- Commitment to providing high-quality patient care and contributing to the development of clinical pharmacy services.

EDUCATION:

A PharmD or bachelor's degree from an accredited College of Pharmacy is required.
Completion of PGY-1 Pharmacy Residency or 3+ years working as a clinical pharmacist in direct patient care is required.
Completion of a PGY-2 Ambulatory Care or Specialty Pharmacy Residency/Fellowship) is preferred.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

An active Pharmacist licensure (in good standing) to practice pharmacy in the state in which you are applying in or and/or the ability to acquire this at the time of hire is required.
Must possess or be able to obtain a State Board of Pharmacy Preceptor’s license by the current state you are applying to within one year of hire.

EXPERIENCE:

2+ years of work experience in ambulatory care pharmacy setting post PGY1 training or 3+ years post PharmD is preferred.

Job Type: Full-time

Pay: $66.35 - $69.71 per hour
