Full Job Description

Job Overview

We are seeking a dedicated and detail-oriented MDS Coordinator / Case Manager to join our healthcare team. In this role, you will be responsible for conducting comprehensive patient assessments, ensuring accurate documentation, and supporting regulatory compliance to optimize patient outcomes. You will collaborate closely with clinical teams and external partners to deliver high-quality, patient-centered care in a dynamic healthcare environment.

Responsibilities

Conduct thorough patient assessments using MDS (Minimum Data Set) tools and ensure accurate, timely documentation in electronic health records (EHR) such as Epic, Cerner, or Athenahealth
Review and analyze medical records, including medical terminology, ICD-10 coding, CPT coding, and clinical documentation to support care planning and reimbursement
Collaborate with an interdisciplinary team (nursing, physicians, social workers, discharge planners) to develop individualized care plans in alignment with Medicare and NCQA standards
Coordinate discharge planning for inpatient, outpatient, hospice, and home care settings to ensure smooth transitions while maintaining HIPAA compliance
Perform utilization review and management to evaluate hospital stays and clinical episodes, supporting appropriate resource use and patient outcomes
Ensure compliance with healthcare regulations related to health information management (HIM), medical records, and clinical documentation improvement
Support accurate coding and documentation for DRGs, ICD-10, and CPT to ensure proper billing and reimbursement

Qualifications

Experience in case management or clinical coordination, preferably in hospital or acute care settings (ICU, trauma, PICU, or NICU experience a plus)
Strong knowledge of Epic EMR or similar systems such as Meditech or eClinicalWorks
Familiarity with managed care processes, Medicare guidelines, utilization review, and NCQA standards
Working knowledge of ICD-10, CPT coding, DRG assignment, and clinical documentation improvement
Background in nursing or allied health preferred; critical care experience is highly valued
Strong understanding of anatomy, physiology, medical terminology, and healthcare compliance (including HIPAA)
Ability to work across inpatient and outpatient settings, including specialty areas such as emergency medicine and hospice care
