Full Job Description

JOB SUMMARY: The Health and Wellness Case Coordinator works with the current residents of San Jose Family Shelter and clients in the AfterCare Program. This position assesses client’s health, behavioral health and develops individualized service plans; connects clients with community resources that promote stability and overall wellbeing. Requires some work on weekends and evening hours.

EDUCATION AND EXPERIENCE:

BA/BS or comparable field experience in social/human service or public health
Minimum one year experience working with case management in a public or non-profit setting.
CPR certified.

Essential Duties and Responsibilities: The following statements are intended to describe the general nature and scope of work being performed by this position. This is not a complete listing of all responsibilities, duties and/or skills required. Other duties may be assigned.

· Coordinate and oversee onsite service provisions with community agencies.

· Schedule monthly TB test appointments for shelter residents and staff as needed.

· Participate in all group and individual intakes to assess client’s needs and facilitate access to health and supportive services.

· Evaluate shelter/aftercare family needs by completing a health assessment to determine their immediate needs, functional capabilities and assistance needed to stabilize each member of the family.

· Plan, organize and conduct workshops and activities for shelter and AfterCare clients every month.

· Conduct case management to shelter/aftercare clients based on health needs.

· In coordination with the Shelter Case Managers, identify program participants.

Oversee the overall progress of the families in the programs, specific to the health of each member.
Maintain accurate, timely and confidential client records, case notes and service transactions into clarity HMIS.
Provide follow-up and re-assessment to maintain the most current information on families, as needed. Intervene in acute situations to meet essential needs when usual supportive service arrangements are not available.
Arrange for timely, appropriate delivery of all services needed for families to maintain the highest possible level of independence.
Advocate for families to obtain needed services or receive attention to urgent health related issues.
Maintain and oversee the shelter’s community garden, including seasonal planting, harvesting and general upkeep to provide a safe, welcoming and therapeutic outdoor space that promotes health, nutrition and wellness for the shelter residents.
Collaborate with team members from other disciplines to determine the best course of service delivery for each family.
Report all instances of suspected child abuse to appropriate agencies.
Attend and participate in all required agency meetings: staff meetings, house meetings, in-service training, and weekly supervision.
Follow all agency protocols regarding safety and professional behavior.
Keep the primary case manager, Program Manager and Program Director up to date on the progress of each resident family.
Maintain current knowledge of services and resources available for homeless families within Santa Clara County.
Other duties in keeping with the scope and nature of the position.

Required Knowledge, Abilities and Skills

Excellent oral and written communication skills
Proficient in Microsoft Teams, Word, Excel and other computer software in the performance of the work.
Active team player with a strong personal work ethic.
Maintain up-to-date knowledge of services and resources available for homeless families.
Maintain HIPPA compliance.
Maintain accurate statistical information of families seen and services provided and provide statistical and monthly reports as required in a timely manner.

· Be familiar with all Shelter policies to respectfully serve the Shelter population and represent the Shelter effectively when working with public agencies.

Physical Ability: Tasks require the ability to exert light physical effort in sedentary light work, but which may involve some lifting, carrying, pushing and/or pulling of objects and materials of light weight (5-10 pounds) to medium (up to twenty-five pounds). Tasks may involve extended periods of time at a keyboard or workstation. Operates assigned office equipment.
