Full Job Description

Overview

We are seeking a dedicated, compassionate, and community-driven Patient Navigator to join our team at the Queer Collective. In this vital role, you will serve as a guiding resource and advocate for our patients, helping them navigate complex healthcare, legal, and social systems. Your proactive approach will empower LGBTQ+ individuals to access essential services, including gender-affirming care, mental health support, and housing assistance. This position offers an exciting opportunity to make a direct, meaningful difference by providing trauma-informed, affirming, and personalized advocacy throughout our patients' healthcare journeys.

Responsibilities

Assist patients in understanding and accessing available medical and community resources, including gender-affirming healthcare, mental health services, and sexual health programs (e.g., PrEP/PEP, HIV/STI testing).
Coordinate appointments, follow-up care, and service planning with LGBTQ+-affirming healthcare providers, social services, and legal advocates.
Facilitate welcoming and culturally competent intake processes for new patients, identifying their unique needs and connecting them with tailored internal and external resources.
Maintain accurate and confidential documentation of patient interactions and care plans within EMR/EHR systems, strictly adhering to HIPAA and privacy standards to protect our community.
Collaborate with local healthcare providers, social workers, and allied organizations to build and maintain a robust, trusted referral network.
Act as a strong advocate for patients who may face systemic barriers, bias, or discrimination when seeking external medical care or social services.

Qualifications

Proven experience in patient navigation, community health, social work, or a related role directly serving the LGBTQ+ community.
Deep understanding of the unique medical, social, and systemic challenges faced by LGBTQ+ individuals, particularly transgender and gender-nonconforming people, and QTPOC (Queer and Trans People of Color).
Familiarity with trauma-informed care practices, harm-reduction models, and patient advocacy.
Knowledge of local LGBTQ+-affirming resources, healthcare networks, and social services.
Excellent, empathetic communication skills with a demonstrated ability to work compassionately and effectively with highly diverse populations. Bilingual abilities are a strong plus.
Familiarity with electronic medical records (EMR/EHR) and strong organizational skills to handle intake assessments efficiently while maintaining strict attention to detail.
