We are seeking an experienced Lead Hospice Registered Nurse (RN) to provide clinical leadership while ensuring the highest standards of hospice care for our patients and families.

What You'll Do

Provide clinical oversight and leadership for hospice nursing staff.
Complete comprehensive patient assessments and develop individualized plans of care.
Coordinate care with physicians, nurse practitioners, social workers, chaplains, home health aides, and interdisciplinary team members.
Supervise and mentor hospice nurses and clinical staff.
Ensure compliance with Medicare Conditions of Participation, state regulations, and agency policies.
Monitor quality assurance and performance improvement initiatives.
Review documentation for accuracy, completeness, and regulatory compliance.
Participate in patient admissions, recertifications, and discharge planning.
Provide education and emotional support to patients and their families.
Participate in on-call rotation as assigned.
Assist with staff orientation, training, and competency evaluations.
Qualifications
Current Texas Registered Nurse (RN) License.
Minimum of 3 years of hospice nursing experience.
Previous leadership or supervisory experience preferred.
Current BLS Certification.
Valid Texas driver's license and reliable transportation.
Strong knowledge of hospice regulations, Medicare guidelines, and documentation standards.
Excellent communication, organizational, and leadership skills.
Compassionate bedside manner with exceptional critical-thinking abilities.

Preferred QualificationsBenefits

Conduct chart audits.

Ability to lift up to 50 pounds with assistance.
Ability to sit, stand, bend, and walk for extended periods.
