Full Job Description

Description: This position is primarily responsible for supporting caregivers and office staff by assisting with the start of care for clients and training caregivers on the use of company software and CNA-level skills.

Qualifications: The Field Supervisor will have a current CNA license in the state of Maryland and at least two years of experience working as a CNA/Medicine Technician. Management and/or supervisory experience is preferred.

Reports to: the Director of Nursing .

Primary Duties:

1. Attends first shift for caregivers going to new clients to introduce caregiver and review care plan with

client and caregiver to ensure proper expectations.

2. Follow-up visit or call with new Client and Caregiver within the first week (48-72 hours) of the start of care to ensure expectations for care are aligned and all is going well.

3. Collaborate with the Care Coordinating Team.

4. Review the Clocking and Clock report weekly to determine who needs training on the proper clock- in/out and task log completion. The Director of Operations will review the Clocking In/Out Report and provide a list of names to the Field Supervisor of those Caregivers who need training on the Clear Care

App for Clocking In/Out and/or properly completing task logs.

5. Visit caregivers in clients' homes to train them on the proper use of the Wellsky App for clocking in and out and troubleshooting any problems. Check in with Credentialing to see if any testing or other information is needed before the visit and obtain information if possible.

6. Provide Medicine Technician services as assigned by care coordinators.

7. Provide CNA services for occasional short shifts or emergency fill-in cases, as the Client Care Coordinator

assigns.

8. Collect updated credentials from Caregivers who are in the field with expired/or about to expire credentials. Also, deliver annual training documents to caregivers in the field as assigned by the Director of HR.

9. call/visit Caregivers to provide support and training when requested. It will be essential and ongoing to work towards building and maintaining relationships and trust with all Caregivers.

10. Provides on-site training, as needed, with counseling, coaching, and educating Caregivers and Clients or, if the need is outside the individual’s scope of experience, makes requests for follow-up from other staff members. If training is provided, document the training in the “Notes” section of Wellsky, and include

the Caregiver name, Client name, and the ‘Activity Tag’ “Caregiver Training.”

11. During training visits, observe the client for any changes in condition or changes in care needs. If

changes are identified, notify the nursing team via “Task” in Wellsky.

12. Update and maintain the Clear Care database to include daily schedule, daily activity logs, and daily

follow-up tasks for Clients and Caregivers that have been visited as well as their own schedule.

13. If requested, attend and contribute to Caregiver training during in-office Orientation for applicants.

14. Completes other tasks as assigned by Agency Administration.

15. Working with VA, assisting with hands-on recruitment and employee engagement.

Basic Hours: 8:30 am to 5:00 pm Monday to Friday but may vary depending on shift start schedules to include client visits during early morning hours or evening hours. Participation in Sunday weekend on call coverage plan.
