Full Job Description

Pharmacist — Link Pharmacy (1st and 2nd Shift )Overview

As a pharmacist in our closed-door compounding pharmacy, you will be responsible for reviewing prescriber orders and compounding, preparing, verifying, and overseeing the dispensing of customized sterile and/or non-sterile medications. We do not serve walk-in retail customers; our patients are served through prescriber orders and direct delivery or shipment, in coordination with physicians, clinics, facilities, and care teams.

You must be fully qualified to evaluate the clinical appropriateness, accuracy, and legality of each order, to perform and verify compounded preparations against the master formulation record, and to consult with prescribers and patients on drug therapy, dosing, interactions, and safe medication use.

The goal is to ensure the safe, accurate, and compliant preparation of compounded medications and to serve as a trusted clinical resource for the prescribers, facilities, and patients we support — delivering maximum value and quality to both the pharmacy and the people we serve.

Responsibilities

Review, compound, prepare, verify, and/or oversee the compounding, preparation, and dispensing of sterile and/or non-sterile pharmaceuticals; perform the final accuracy and clinical check of compounded preparations against the master formulation record and prescription.
Review and execute prescribers' orders, checking each for clinical appropriateness, accuracy, completeness, and legality; make decisions regarding generic and product substitution where permitted.
Compound preparations using correct calculations, ingredients, dosage forms, and techniques for each individual patient; assign and document appropriate beyond-use dates (BUD).
Maintain and follow master formulation records and compounding records; ensure accurate documentation of every preparation.
Consult with prescribers, facility staff, and patients on drug indications and contraindications, dosing, drug interactions, side effects, and stability/storage; provide other drug information as appropriate.
Oversee the acquisition, control, and disbursement of drugs and chemicals, including controlled substances, in compliance with DEA and state requirements.
Maintain control over receiving, stocking, and labeling of medications, chemicals, and supplies, and monitor their condition to prevent expiration, contamination, or deterioration.
Establish and maintain storage and recordkeeping systems that ensure the safekeeping, traceability, and integrity of pharmaceuticals and compounding components.
Organize pharmacy inventory and workflow to support accurate, efficient, and safe compounding and dispensing operations.
Perform and document quality control and quality assurance activities, including environmental monitoring, equipment and facility checks, and recordkeeping required by USP standards and federal, state, and local regulations.
Maintain compliance with USP <795> (non-sterile compounding), USP <797> (sterile compounding), and USP <800> (hazardous drugs) as applicable, including proper aseptic technique and use of the cleanroom/controlled environment.
Provide education, guidance, and training to pharmacy technicians, students, interns, and/or trainees during the course of work, and monitor performance.
Follow established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and infection-control standards.
Communicate clearly with prescribers, facilities, and patients to clarify orders and resolve discrepancies, and offer accurate drug information and advice.
Comply with all applicable legal rules, regulations, and procedures, including State Board of Pharmacy, DEA, and USP requirements.
Perform miscellaneous job-related duties as assigned.

Requirements and Skills

Proven experience as a pharmacist; compounding pharmacy experience strongly preferred.
Thorough understanding of pharmaceutical calculations, dosage administration and measurement, chemical compounds, dosage forms, and compounding techniques.
Working knowledge of USP <795>, <797>, and <800>, aseptic technique, and cleanroom/controlled-environment practices (sterile compounding experience preferred where applicable).
Strong organizational skills and meticulous attention to detail and documentation.
Excellent communication skills with a prescriber- and patient-oriented approach.
Valid license to practice pharmacy in Texas.
Comfortable working evening shifts. prior closed-door, compounding, sterile/IV, or hospital pharmacy experience

Conditions of Employment

May require discipline-specific licensure and/or professional certification, as specified by the department in the preferences.
Must hold and maintain an active pharmacist license in good standing in Texas.
Must pass a pre-employment criminal background check.
May be subject to a credentialing process; must maintain credentialed status, if applicable.
Must maintain license and certification status throughout employment.
