Full Job Description

Summary:

The Psychiatric Nurse provides nursing and care coordination support for HOPE Community Medicine’s Behavioral Health program. This role assists Behavioral Health providers with patient care, medication management workflows, telehealth coordination, screenings, documentation, and follow-up services to ensure patients receive compassionate, high-quality mental health care in a supportive clinical environment. This is a hybrid position that combines in-clinic patient care and telehealth support. The position requires regular travel to HOPE Community Medicine's Center and Carthage clinic locations to support Behavioral Health services and may require occasional travel to other clinic locations as operational needs arise.

Expectations:

· Assist Behavioral Health providers with daily clinical and patient care workflows.

· Support psychiatric medication refill workflows, prior authorizations, and patient follow-up coordination.

· Maintain compliance with HIPAA, HRSA, FQHC, and HOPE Community Medicine clinic policies and procedures.

· Provide compassionate, patient-centered support to individuals experiencing behavioral health conditions.

· Work collaboratively with providers, counselors, nursing staff, pharmacies, and care coordination teams.

· Work flexible or extended hours when necessary to support clinic operations.

· Travel routinely between the Center and Carthage clinic locations to provide clinical, direct patient care, telehealth coordination, and operational support.

· Abide by Rules of Confidentiality and policies outlined in the Employee Handbook.

· Demonstrate awareness of and compliance with HOPE Community Medicine’s mission.

· Perform additional duties as assigned by Behavioral Health leadership.

Job Duties:

· Complete patient intake, screenings, medication reconciliation, vitals, and documentation.

· Assist providers with Behavioral Health patient flow and follow-up care coordination.

· Monitor refill requests, patient messages, prior authorizations, and Behavioral Health tasks within the EMR.

· Support telehealth and in-person Behavioral Health appointments.

· Educate patients on medications, treatment plans, follow-up appointments, and available resources.

· Assist with obtaining outside medical records, hospital documentation, and behavioral health referrals.

· Assist in identifying patients needing additional support, crisis intervention, or community resources.

· Maintain complete, accurate, and timely documentation within the electronic medical record system.

· Assist with behavioral health screenings and workflow management.

· Coordinate communication between Behavioral Health, Primary Care, pharmacies, and outside providers.

· Support Behavioral Health quality improvement initiatives and Behavioral Health operational workflows.

· Provide responsive support to staff and patients across all clinic sites.

Required Qualifications:

· Current Texas LVN or RN license in good standing required.

· Current Basic Life Support (BLS) certification.

· Behavioral Health, Psychiatry, or Mental Health experience preferred.

· Familiarity with eClinicalWorks (eCW) or similar EMR systems preferred.

Skills:

· Knowledge of psychiatric medications and behavioral health workflows helpful.

· Excellent verbal and written communication skills.

· Strong organizational, prioritization, time management abilities, and problem-solving skills.

· Ability to work independently while maintaining effective communication with remote and on-site team members.

· Ability to manage multiple tasks in a fast-paced healthcare environment.

· Ability to maintain professionalism and compassion when working with sensitive patient situations.

· Willingness to learn, grow, and contribute to the continued development of HOPE Community Medicine’s Behavioral Health operations and workflows.

Physical Requirements:

· Must be able to remain in a stationary position for extended periods.

· Occasionally required to move about inside office or clinic sites.

· May require lifting equipment or supplies weighing up to twenty (20) pounds.

· Ability to communicate effectively in person, by phone, and electronically.
