Summary
The Long Term Care RN is responsible for directing and supervising resident care consistent with the Iowa Nursing Practice Code, Federal, State, and Sioux Center Health standards of care while promoting each resident’s highest practicable physical, mental, and psychosocial well-being.

RN Essential Functions
Utilize the “nursing process” (i.e. ongoing assessment, nursing diagnosis, planning, intervention, and evaluation) in the practice of nursing.
To assess the health status of each resident.
To formulate a nursing diagnosis based on the analysis of data from the nursing assessment;
To plan nursing care which includes determining goals and priorities for activities based on the nursing diagnosis;
To develop nursing interventions implementing the plan of care; and
To evaluate the individual resident’s status in relation to established goals and the plan of care.
Recognize and understand the legal implications of accountability including, but no limited to the following:
Performing or supervising those activities and functions which require the knowledge and skill level of a registered nurse and seeking assistance when activities and functions are beyond the licensee’s scope of preparation;
Using professional judgment supervising unlicensed assistant personnel;
Using professional judgment in assigning and delegating activities and functions to unlicensed assistant personnel;
Supervising, among other things, includes any or all the following:
Direct observation of any activity or function;
Assumption of overall responsibility for assessing, planning, implementing, and evaluating nursing care;
Delegation of nursing tasks while retaining accountability;
Determination that nursing care being provided is adequate and delivered appropriately.
Executing the regimen prescribed by the physician while exercising professional judgment in accordance with minimum standards of nursing practice.
Performs, skillfully and proficiently, the following duties:
Reads report from previous shift and writes report for following shift.
Reports and explains all pertinent resident care information to C.N.A.’s.
Supervises all meals and nourishment or assure proper diet is served, nutrition and hydration needs are met, and residents are assisted as necessary.
Makes rounds as necessary, assessing unstable residents.
Prepares and administers medications, nutrition, fluids and medication, and treatments as ordered.
Documents all medications, IV fluids, and treatments as administered.
Supervises any restraint application and/or removal.
Supervises prompt response to resident’s call lights.
Orders medications as necessary.
Notifies pharmacy of changes/additions to resident’s drug regimen.
Receives medication delivery from pharmacy and checks and documents all drug received.
Supervises all resident care and assists with direct care as needed.
Observes, evaluates, reports, and documents all changes in resident’s conditions every two hours or as needed.
Assists in the managerial responsibilities (interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems). Performs other duties and responsibilities as assigned.
LPN Essential Functions
Participate in the “nursing process” (i.e. ongoing assessment, nursing diagnosis, planning, intervention, and evaluation), consistent with accepted and prevailing practice, by assisting a registered nurse or physician. The LPN may assist in monitoring, observing, and reporting reactions to interventions. Refrain from performing any activity requiring the knowledge and skill of a registered nurse including:
The initiation of an assessment related to procedures/therapies requiring the knowledge or skill level of a registered nurse;
The initiation of intravenous solutions, intravenous solutions, intravenous medications and blood components;
The administration of medicated intravenous solutions, intravenous medications and blood components;
The initiation or administration of medications requiring the knowledge or skill level of a registered nurse.
Engage in, under the supervision of a registered nurse, a limited scope of practice in intravenous therapy as follows:
- Add intravenous solutions without medications to established peripheral intravenous sites;
- Regulate the rate of non-medicated intravenous solutions;
- Administer maintenance doses of analgesics via resident-controlled analgesic pump set at a lock-out interval;
- Discontinue intravenous therapy.
Perform intravenous therapy procedures as outlined in Iowa Code, Chapter 6, Nursing Practice for Registered Nurses/Licensed Practical Nurses following completion of a board-approved intravenous therapy certification course offered by a board-approved provider of education. Recognize and understand the legal implications of accountability including, but not limited to the following:
- Performing those activities and functions which require the knowledge and skill level of a licensed practical nurse and seeking assistance when activities and functions are beyond the licensee’s scope of preparations;
- Accepting responsibility for performing assigned and delegated functions and informing the registered nurse when assigned and delegate functions are not executed.
- Executing the medical regimen prescribed by a physician. In executing the medical regimen as prescribed by the physician, the licensed practical nurse will exercise prudent judgment in accordance with minimum standards of nursing practice. If the medical regimen prescribed by the physician is not carried out based on the LPN’s prudent judgment, accountability will include but need not be limited to the following:
- Timely notification of the physician who prescribed the medical regimen that order(s) was not executed and reason(s) for same.
- Documentation on the medical records the physician was notified and reason(s) for not following order(s).
Supervise unlicensed assistant personnel under the provisions of the Iowa Code. Supervision, among other things, includes any or all of the following:
- Direct observation of a function or activity;
- Delegation of nursing tasks while retaining accountability;
- Determination that nursing care being provided is adequate and delivered appropriately.
- A licensed, practical nurse may serve as a charge nurse under the supervision of a registered nurse.
Assists in the managerial responsibilities (interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems). Performs other duties and responsibilities as assigned.
