Full Job Description

Job Title: Hospice Triage Nurse (7-On / 7-Off)

Location: Scottsdale- East Valley
Schedule: 7 days on / 7 days off (typically 12-hour shifts)
Employment Type: Full-Time
Reports To: Clinical Manager / Director of Nursing

Position Summary

The Hospice Triage Nurse provides clinical assessment, guidance, and support to patients, caregivers, and healthcare providers during assigned coverage hours. This role is responsible for assessing patient symptoms, determining urgency of care needs, and coordinating appropriate clinical interventions while ensuring compliance with Arizona nursing standards and organizational policies.

Key Responsibilities

Provide assessments to patients and caregivers
Assess symptoms, pain, and changes in condition using established triage protocols
Determine appropriate level of care and initiate interventions as needed
Provide patient and caregiver education, reassurance, and emotional support
Coordinate care with on-call nurses, physicians, and interdisciplinary team members
Document all assessments, recommendations, and interventions accurately and timely in the electronic medical record (EMR)
Escalate urgent or emergent situations according to clinical guidelines
Support continuity of care by communicating updates to the care team
Ensure compliance with state, federal, and organizational regulations
Participate in quality improvement and ongoing education initiatives

Qualifications

Required:

Current, unrestricted Registered Nurse (RN) license in the state of Arizona
Minimum of 2 years of recent clinical nursing experience, hospice preferred
Strong assessment and critical-thinking skills
Excellent communication and documentation skills
Ability to work independently during extended coverage periods

Preferred:

Hospice, home health, palliative care, emergency, or triage experience
Experience with telephone triage or telehealth platforms
Familiarity with EMR systems and remote documentation
Knowledge of Arizona nursing scope of practice

Skills & Competencies

Strong clinical judgment and decision-making abilities
Calm, compassionate communication style
Ability to manage multiple calls and priorities efficiently
Comfort working evenings, nights, weekends, and holidays as part of the 7-on / 7-off schedule
High level of professionalism, accountability, and reliability

Schedule Details

7 consecutive days on, followed by 7 consecutive days off
Typical shifts are 5pm-8am shifts
Includes nights, weekends, and holidays during assigned week

Physical & Work Requirements

Ability to sit for extended periods and work at a computer/telephone
Reliable high-speed internet and quiet work environment (if remote)
Ability to respond quickly and appropriately in urgent situations
