Job Summary

We are seeking a dedicated and energetic Physical Therapist (PT) to join our dynamic healthcare team. In this role, you will provide expert rehabilitation services to patients in our skilled nursing facilities. Your primary focus will be to develop personalized treatment plans, facilitate patient recovery, and improve mobility and quality of life. You will collaborate closely with healthcare professionals to ensure comprehensive patient care and adhere to clinical documentation standards. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working in a supportive, fast-paced environment.

Responsibilities

Assess patients’ physical conditions through detailed evaluations, including physiology and anatomy knowledge, to determine appropriate treatment strategies.
Develop and implement customized treatment plans that align with patient goals and clinical best practices, including discharge planning and care plans.
Administer therapeutic interventions such as physiotherapy techniques, trigger point therapy, and neurology-based treatments to promote healing and mobility.
Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate patient care and optimize outcomes.
Maintain accurate and thorough documentation of patient progress using electronic health records (EHR) systems in compliance with clinical documentation standards.
Support patients in skilled nursing facilities.
Educate patients and their families on treatment procedures, exercises, and self-care strategies to enhance rehabilitation success.

Skills

Strong knowledge of physiology, anatomy, orthopedics, geriatrics, and neurology to inform effective treatment approaches.
Proven experience in outpatient rehabilitation centers, inpatient hospital settings, or skilled nursing facilities.
Familiarity with medical terminology and clinical documentation standards necessary for accurate record keeping.
Ability to develop comprehensive treatment plans tailored to individual patient needs in the adult geriatric population.
Excellent client communication skills to motivate patients and explain complex procedures clearly.
Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for efficient documentation management.
Knowledge of discharge planning processes and working within outpatient or inpatient care frameworks to ensure seamless patient transitions.
