Full Job Description

The Patient Navigator will engage unsheltered persons, persons living with serious mental illness and/or substance use disorders, and other priority populations with linkage to HCV treatment and cure, mental health, substance use, and social supportive services. The Patient Navigator will use a peer-led approach to ensure engagement in and adherence to treatment recommendations and other needs as identified by the program team. Specifically, the Patient Navigator ensures the recommended treatment services for persons living with HCV and referrals to mental health and substance use disorder prevention, treatment and care services for completed across the Corridor of Care, which spans from Washington, DC, to Baltimore City, MD.

Responsibilities: • Engages in outreach activities to identify vulnerable populations living with HCV as assigned by the Program Coordinator. • Supports Community Health Workers with HCV, HIV and other sexually transmitted infection (STI) screenings and applicable linkage to care services. • Provides patient navigation to assigned caseload and ensures treatment recommendations and service referrals are adhered to among individuals living within the Corridor of Care. • Utilizes motivational interviewing to support with linkage to care and referral follow through • Utilizes unique engagement strategies to keep vulnerable individuals engaged in service navigation and treatment recommendations. • Adheres to program protocols and procedures as directed by the Program Coordinator. • Monitor initial program implementation and track project activities as outlined in the internal project plan. • Use local epidemiology data to guide teamwork and program activities. • Ensures linkages to behavioral health and substance use are well coordinated and tracked. • Support community engagement and surveillance activities based on assignments and community trends. • Enters documentation as outlined by the organization’s standards • Adhere to program reporting requirements and data collection • Supports agency-wide initiatives and programs. • Other duties as assigned.

Experience and Education: ● High School Diploma or GED required. Persons with certifications in Peer Educator or Patient Navigation are encouraged to apply ● Minimum two (2) years of experience in customer service in a healthcare, non-profit, or social service setting. ● Experience working with vulnerable populations (unhoused and individuals living with serious mental illness)

Knowledge, Skills & Abilities: ● Ability to work with individuals living with substance use, behavioral health, and unhoused challenges. ● Ability to demonstrate sensitivity, empathy, and understanding of vulnerable populations. ● Ability to use computer software systems ● Familiarity with community needs in DC, PG County, and Baltimore ● Ability to perform 80% of field outreach and mobile outreach. ● Ability to foster a cooperative work environment and provide caring and non-judgmental client-centered services. ● Bilingual English/Spanish and/or other languages strongly preferred
