Full Job Description

Job Summary
We are seeking a compassionate and dedicated Hospice Aide who is a vital paraprofessional member of the interdisciplinary care team, working under the supervision of a Registered Nurse (RN) to provide essential personal care and comfort to patients. This role includes observing and reporting changes in a patient’s condition, ensuring their personal needs are met, and maintaining a safe and comfortable environment. The Hospice Aide is assigned to patients in a manner that prioritizes quality, continuity, and safety of care.

Essential Job Functions & Responsibilities

Key responsibilities include, but are not limited to:

Personal Care: Provides bathing, oral hygiene, back rubs, shampoos, dressing, and assistance with toileting as assigned.
Mobility Support: Assists with ambulation, exercises, and range of motion per care plan.
Nutritional Support: Assists with feeding and ensures hydration.
Skin & Comfort Care: Observes and prevents skin breakdown over bony prominences.
Medication Assistance: Provides reminders for self-administered medications as per state regulations.
Observes and reports significant changes in patient condition to the Case Manager.
Maintains awareness of the patient’s environment, including interactions with family members or caregivers.
Takes and records temperature, pulse, respiration, and blood pressure as required.
Ensures the patient’s living space is clean and orderly.
Uses equipment safely and appropriately (e.g., footstools, side rails, oxygen tanks).
Provides respite care for caregivers when needed.
Adheres to all documentation standards and care procedures.
Participates in quality assessment and performance improvement initiatives.
Respects patient dignity, privacy, and cultural values.

The above duties represent core responsibilities but are not exhaustive. Additional tasks may be assigned based on patient needs and organizational requirements.

Position Qualifications

Meets the training and certification requirements as mandated by state regulations.
Completion of 75 hours of classroom and supervised practical training, including 16 hours of hands-on experience before direct patient care.
Must be at least 18 years old.
Ability to read, follow written instructions, and accurately document patient care.
Understanding of hospice philosophy and comfort in providing end-of-life care.
Strong interpersonal skills, including empathy and patience.
CPR certification (current and maintained).
Must possess a valid driver’s license and an insured, reliable vehicle.
Ability to work independently with minimal supervision.
Demonstrates professionalism, good hygiene, and tact in patient interactions.

Job Limitations

The Hospice Aide may not perform duties considered to be the practice of nursing, including:

Administering medications
Taking physician orders
Performing sterile procedures

Working Conditions

Home and community-based environment
Exposure to infectious diseases
Frequent travel required for patient visits

Details: PRN/per diem:

Compensation: $35 per visit; $24/hour for meetings and trainings; plus mileage

Pay: $35.00 per hour
