Full Job Description

Job Overview

NextCare Providers LLC is seeking an experienced and highly organized Licensed Practical Nurse (LPN) to serve as Assistant Director of Nursing (ADON) within our home health agency.

This is a full-time hybrid clinical and administrative leadership position combining direct patient care, nursing staff management, clinical compliance, quality assurance, and operational support.

The ADON will work closely with the Director of Nursing (DON) and administrative team to oversee field nursing operations, support nursing staff, address clinical and staffing concerns, and help ensure continuity and quality of patient care.

The position includes approximately 30 patient visits per week, Monday through Friday, in addition to two designated office days for clinical administrative responsibilities. The ADON will also participate in an on-call rotation with the Director of Nursing.

Candidates located in or familiar with the West & East Valley are preferred due to current patient and operational needs.

Compensation & Schedule

Salary: $75,000 annually
Employment Status: Full-Time
Patient Visits: Approximately 30 visits per week, Monday–Friday
Office Requirement: 2 days per week
Weekend On-Call: Rotating with the Director of Nursing
Weekday On-Call: May be required when another clinical backup is unavailable
Location: Phoenix metropolitan area, with preference for candidates located in the West Valley

This position requires reliable transportation and regular travel to patient residences and other locations as necessary.

Key ResponsibilitiesPatient Care & Field Operations

Complete approximately 30 skilled nursing visits per week, Monday through Friday.
Provide nursing services within the LPN scope of practice and in accordance with patient plans of care and physician orders.
Maintain accurate and timely clinical documentation in WellSky/Kinnser.
Communicate changes in patient condition or other clinical concerns through the appropriate chain of command.
Coordinate with physicians/PCPs, facilities, patients, and families regarding patient-care needs and concerns.
Assist with continuity of care when staffing or clinical concerns arise.

Nursing Staff Management

Assist with the day-to-day management and support of field nursing staff.
Assist with nurse staffing and patient coverage.
Identify nursing performance, documentation, attendance, communication, or patient-care concerns and work with appropriate leadership to troubleshoot and resolve them.
Follow up with nurses regarding outstanding documentation, clinical concerns, compliance requirements, and corrective actions.
Provide guidance and support to nurses while escalating matters requiring DON or administrative involvement.
Assist with nurse orientation, training, and ongoing education.

Clinical Administration & Compliance

The ADON will spend approximately two days per week in the office supporting clinical and administrative operations, including:

Clinical compliance monitoring
Quality Assurance (QA)
Review of nursing documentation
Nurse follow-up
Administration and coordination of nursing in-service training
Clinical staff education and compliance follow-up
Identification and correction of documentation deficiencies
Assistance with internal and external audit preparation
Monitoring adherence to agency clinical policies and procedures

Incident Reporting & Fact-Finding

Prepare and assist with Incident Reports (IRs) involving patient care or nursing-related incidents.
Conduct and respond to clinical fact-finding inquiries.
Gather relevant information from nursing staff and other involved parties.
Document findings objectively and accurately.
Communicate significant findings to the DON and appropriate administrative leadership.
Assist with corrective follow-up and prevention efforts when appropriate.

Care Coordination & Communication

Communicate professionally with PCPs, healthcare facilities, patients, families, nurses, and administrative staff.
Follow up on physician orders and clinical concerns.
Maintain timely communication regarding changes affecting patient care.
Serve as a clinical resource for field nurses within the scope of the position.
Maintain HIPAA compliance and confidentiality at all times.

On-Call Responsibilities

Participate in a rotating weekend on-call schedule with the Director of Nursing.
Provide clinical and operational support to field nursing staff while on call.
Assist with urgent staffing or patient-care concerns.
Provide weekday on-call coverage when another designated clinical backup is unavailable.
Appropriately escalate matters requiring DON, provider, emergency, or administrative intervention.

Required Qualifications

Current and unrestricted Arizona LPN license
Current CPR/BLS certification
Valid Arizona Fingerprint Clearance Card or ability to obtain required clearance
Nursing experience in a clinical setting
Strong clinical judgment within the LPN scope of practice
Strong leadership and problem-solving abilities
Excellent written and verbal communication skills
Ability to manage and provide guidance to nursing staff
Strong organizational and time-management skills
Ability to prioritize multiple clinical and administrative responsibilities
Strong attention to detail and documentation accuracy
Comfortable communicating with physicians, facilities, patients, families, and nursing staff
Comfortable using electronic medical records and Microsoft applications
Reliable transportation and ability to travel throughout assigned service areas
Ability to participate in the required on-call rotation

Preferred Qualifications

Previous home health experience
Previous nursing leadership, supervisory, lead nurse, or clinical administrative experience
Experience with WellSky/Kinnser
Experience with DDD and/or AHCCCS services
Experience with quality assurance and clinical compliance
Experience preparing or reviewing incident reports and fact-finding documentation
Experience training, mentoring, or supervising nurses
Familiarity with EVV requirements
Proficiency with Microsoft Teams, Outlook, Word, and Excel
Residence in or familiarity with the West Valley
