We are looking for a compassionate, organized, and relationship-focused Admissions Coordinator to serve as the first point of contact for prospective residents, families, hospitals, and referral partners.

Position Summary

The Admissions Coordinator is responsible for managing the admissions process from referral through admission while collaborating with hospitals, physicians, case managers, families, and our interdisciplinary team. This role is instrumental in ensuring timely admissions, exceptional customer service, and a seamless experience for new residents and their loved ones.

Responsibilities

Respond promptly to referrals from hospitals, physicians, and community partners.
Review referral documentation to determine admission eligibility in collaboration with clinical leadership.
Coordinate admissions with nursing, therapy, social services, and other departments.
Communicate clearly and compassionately with residents and families throughout the admission process.
Build and maintain positive relationships with hospitals, case managers, discharge planners, and referral sources.
Conduct facility tours for prospective residents and families.
Verify insurance coverage and coordinate with the business office regarding financial information as needed.
Complete admission paperwork accurately and ensure all required documentation is obtained.
Maintain referral and admission records in accordance with company policies and regulatory requirements.
Participate in community outreach and marketing initiatives to support census growth.
Assist with weekend or after-hours admissions as needed.

Qualifications

High school diploma or equivalent required; associate's or bachelor's degree preferred.
Experience in admissions, healthcare marketing, customer service, or long-term care preferred.
Knowledge of Medicare, Medicaid, managed care, and insurance verification is preferred.
Excellent communication and interpersonal skills.
Strong organizational skills with the ability to manage multiple priorities.
Proficiency with Microsoft Office and electronic medical record systems.
Ability to work collaboratively with interdisciplinary teams while maintaining confidentiality.
