Company Overview

As a trusted non-profit organization, it offers a wide array of social services aimed at fostering self-sufficiency and strengthening the community through innovative programs and community collaboration.

Job Summary

We are seeking an energetic and detail-oriented SOAR (SSI/SSDI Outreach, Access and Recovery) Case Manager to join our team. In this vital role, you will coordinate comprehensive case management services for individuals experiencing or at risk of homelessness, focusing on those with mental health conditions, substance use disorders, and complex medical needs. Your efforts will empower clients to access critical benefits, healthcare resources, and community supports, ultimately guiding them toward stability and independence.

Duties

Conduct thorough intake assessments to evaluate clients’ needs, strengths, and eligibility for benefits and services
Develop individualized care plans that include discharge planning, community resource coordination, and behavioral health support
Provide ongoing client engagement through case management activities that promote recovery and self-sufficiency
Facilitate access to medical records, electronic health records (EHR), and ensure compliance with HIPAA regulations
Collaborate with healthcare providers in hospital settings—including Level I Trauma Centers, Level II trauma centers, ICU units, emergency departments, pediatrics, and inpatient units—to coordinate post-discharge care and outpatient services
Assist clients with navigating social services such as addiction counseling, mental health counseling, managed care programs, and outpatient clinics
Maintain detailed documentation of client interactions within EMR/EHR systems while ensuring confidentiality and accuracy
Prioritize tasks effectively to manage a diverse caseload involving inpatient, outpatient, post-acute care, and community-based services

Experience

Proven experience working in hospital environments such as Level I Trauma Centers or Level II trauma centers; ICU or emergency medicine experience highly preferred
Background in acute care settings including pediatrics, mental health facilities, or post-acute care programs
Knowledge of medical terminology, physiology, diagnostic evaluation procedures, and discharge planning processes
Experience working with individuals with disabilities or complex behavioral health conditions from diverse cultural backgrounds
Demonstrated ability in clinical case management, utilization management, social work practices, and patient service delivery
Familiarity with electronic health records (EHR), HIPAA compliance standards, and medical records management
Strong skills in client communication, task prioritization, working with families and communities affected by substance use disorders or mental health issues
Certification in basic life support (BLS), CPR is preferred; experience working with children or individuals with mental health conditions is a plus
