We are seeking a compassionate, experienced, and dedicated Clinical Director / Director of Patient Care Services to lead our clinical operations and ensure the delivery of high-quality home health services.

Position Summary

The Clinical Director is responsible for the overall direction and management of the agency’s clinical services.

This position establishes, implements, and evaluates goals and objectives that support high standards of care and align with the agency’s mission and philosophy.

The Clinical Director also serves as the Clinical Manager for the purpose of meeting Medicare Conditions of Participation.

Essential ResponsibilitiesClinical Leadership and Patient Care

Coordinate and oversee all direct and indirect patient services provided by clinical personnel.
Provide leadership, guidance, and support to Clinical Supervisors, Nursing Supervisors, coordinators, and clinical staff.
Assist clinical leadership with team management, staffing, and care planning.
Support clinical personnel with the assessment, planning, implementation, and evaluation of patient and family/caregiver care.
Oversee all patient care services and clinical personnel.
Make appropriate patient and personnel assignments.
Coordinate patient care and referrals.
Ensure patient needs are continually assessed.
Ensure individualized plans of care are developed, implemented, reviewed, and updated.
Promote the safe, effective, and high-quality delivery of home health services.

Staff Management and Development

Participate in hiring, evaluating, counseling, and terminating agency personnel.
Evaluate the performance of Clinical Supervisors and Nursing Supervisors.
Help supervisors strengthen their skills in evaluating and supporting clinicians.
Conduct clinician performance evaluations annually or more frequently when needed.
Develop, implement, and evaluate the orientation program for new employees.
Conduct or delegate new-employee orientation.
Plan and implement in-service training and continuing education programs based on staff needs.
Stay informed about developments in nursing and home health care and share relevant information with agency personnel.

Compliance and Quality Improvement

Ensure clinical records comply with local, state, and federal laws and regulations.
Maintain compliance with Medicare Conditions of Participation and ACHC Home Care standards.
Ensure all personnel meet applicable licensure and certification requirements.
Participate in the agency’s performance improvement program.
Evaluate productivity and organizational performance through quarterly and annual reviews.
Maintain expertise in clinical decision-making, quality management, and performance evaluation.
Assist with the development, recommendation, implementation, and administration of agency policies and procedures.

Operations and Administration

Assist the Executive Director/Administrator with preparing and administering the agency’s budget.
Monitor operational indicators, including census and service volume, to identify potential effects on staffing, revenue, and expenses.
Oversee the maintenance of clinical records, reports, statistics, and other documentation used to evaluate agency activities.
Ensure adequate supplies and equipment are available for patient care.
Assist with developing agency goals and strategic priorities.
Promote home health referrals throughout the healthcare community.
Serve as Acting Executive Director/Administrator when the Executive Director/Administrator is absent.
Perform other duties as assigned by the Executive Director/Administrator.

Required Qualifications

Candidates must meet the following requirements:

Hold a current license as a Registered Nurse or physician in the applicable state.
Possess a bachelor’s degree in nursing from an accredited program or a medical degree. A master’s degree in healthcare or a related field is preferred.
Demonstrate the ability to supervise and lead professional and administrative personnel.
Possess strong clinical decision-making and quality-management skills.
Understand business management, governmental regulations, Medicare requirements, and ACHC standards.
Demonstrate excellent observation, verbal communication, and written communication skills.
Be able to interact professionally and tactfully with patients, families, referral sources, customers, and community partners.
Hold a valid driver’s license.
Have access to a reliable vehicle that is properly insured and in good working condition.

Candidates must also meet one of the following experience requirements:

At least three years of management experience within the past five years in a home health or related healthcare organization, including at least one year in a supervisory or administrative role; or
Be a Registered Nurse with at least four years of experience within the past five years in a home health agency, primary care clinic, or healthcare facility, including at least one year in a supervisory or administrative role.

Exceptions may apply to individuals previously approved for the position through program flexibility issued before April 1, 1995.
